毫火针疗法配合针刺三阴交对原发性痛经患者血清β-EP、ET-1水平的影响

摘　　要：目的:探讨毫火针疗法配合针刺三阴交对原发性痛经患者血清β-内啡肽(β-EP)、内皮素(ET-1)水平的影响。方法:以2017年8月—2018年8月我院针灸科门诊收治的76例原发性痛经患者为研究对象。随机将其分为对照组(n=38)和研究组(n=38),其中对照组给予针刺三阴交治疗,研究组在此基础上加用毫火针疗法进行治疗,两组均治疗3个月。比较两组治疗前后视觉模拟评分(VAS)、健康状况调查简表(SF-36)评分、治疗前后血清性激素、血清β-EP、ET-1、前列腺素E2(PGE2)水平及临床疗效。结果:治疗后与治疗前相比,两组VAS评分均降低,SF-36评分均升高,且研究组VAS评分显著低于对照组,SF-36评分显著高于对照组(P <0.05);治疗后两组FSH、LH水平无显著性差异(P> 0.05);治疗后E2水平显著低于治疗前,P水平显著高于治疗前,且研究组E2水平低于对照组,P水平高于对照组(P <0.05);治疗后两组血清β-EP水平较治疗前均升高,ET-1、PGE2水平与治疗前比较均降低,且研究组血清β-EP水平显著高于对照组,ET-1、PGE2水平显著低于对照组(P <0.05);研究组的总有效率为94.74%(36/38),显著高于对照组的73.68%(28/38)(P <0.05)。结论:毫火针疗法配合针刺三阴交治疗原发性痛经临床疗效确切,能够调节患者血清性激素水平及血清β-EP、ET-1、PGE2水平,从而缓解患者痛经症状,提高生活质量。Objective: To investigate the effects of filiform fire needling therapy combined with needling Sanyinjiao( SP6) on the serum levels of β-endorphin( β-EP) and endothelin( ET-1) in patients with primary dysmenorrhea. Methods: A total of 76 patients with primary dysmenorrhea were randomly divided into the control group( n = 38) and the research group( n = 38). The control group was treated with needling SP6;on which basis,the research group was also treated with filiform fire needling therapy. Both groups were treated for3 months. The scores of VAS and SF-36,the levels of serum sex hormones and serum β-EP,ET-1,prostaglandin E2( PGE2) and clinical efficacy before and after the treatment were observed and compared between the two groups. Results: The scores of VAS were decreased and the scores of SF-36 were increased after the treatment,the changes of the research group were more significant than those in the control group( P < 0. 05).There were no statistical differences in the levels of FSH and LH between the two groups after the treatment( P > 0. 05). The level of E2 was obviously lower and the the level of P was obviously higher after the treatment than that before the treatment in the two groups;of which the improvements in the research group were more significant( P < 0. 05). The level of serum β-EP was higher and the levels of ET-1 and PGE2 were lower after the treatment than those before the treatment in the two groups,of which the improvements in the research group were more significant( P < 0. 05). The effective rate was 94. 74%( 36/38) in the research group,which was obviously higher than 73. 68%( 28/38) in the control group( P < 0. 05). Conclusion: Filiform fire needling therapy combined with needling SP6 has a clinically positive effect on primary dysmenorrhea. It can regulate the levels of serum sex hormones and β-EP,ET-1 and PGE2,thereby alleviating dysmenorrhea symptoms and improving life quality of the patients.
