摘 要:目的研究老年2型糖尿病(type 2 diabetes mellitus,T2DM)住院患者血红蛋白(hemoglobin,Hb)与骨质疏松的关系。方法纳入首都医科大学宜武医院内分泌科住院的老年T2DM病例498例,测量患者的骨密度,分为骨质疏松组(160例)和非骨质疏松组(338例)。测定Hb、空腹血糖(fasting blood glucose,FBG)、糖化血红蛋白(glycosylated hemoglobin A1c,HbA1c)、25羟基维生素D3(25-hydroxyvitamin D3,25-OHD3)、白细胞介素6(interleukin-6,IL-6)及C反应蛋白(C-reactive protein,CRP)等。结果骨质疏松组体质量指数(body mass index,BMI)、25-OHD3、肾小球滤过率(estimated glomerular filtration rate,eGFR)、Hb水平显著低于非骨质疏松组(P<0.001),而年龄显著高于非骨质疏松组(P=0.017)。骨质疏松组患者贫血患病率(25.00%)显著高于非骨质疏松组(16.57%)(P=0.026)。多元Logistic回归分析显示,女性、BMI、Hb为骨质疏松的独立影响因素(分别为OR=3.10,P<0.001;OR=0.86,P<0.001;OR=0.90,P=0.018)。结论Hb水平与老年T2DM患者骨质疏松的发生显著相关。Objective To study the association between hemoglobin(Hb)and osteoporosis in elder patients with type 2 diabetes mellitus(T2DM).Methods A total of 498 elder people with T2DM who admitted to the Department of Endocrinology,Xuanwu Hospital of Capital Medical University were included in the study.The participants were divided into osteoporosis group(n=160)and non-osteoporosis group(n=338)according to the measurement of the bone mineral density.The levels of Hb,fasting blood glucose(FBG),glycosylated hemoglobin A1c(HbA1c),25-hydroxyvitamin D3(25-OHD3),interleukin-6(IL-6),and C-reactive protein(CRP)were measured.Results The body mass index(BMI),25-OHD3,estimated glomerular filtration rate(eGFR),and Hb levels in the osteoporosis group were significantly lower than those in the non-osteoporosis group(P<0.001),but age was much higher in the osteoporosis group(P=0.017).The prevalence of anemia was significantly higher in the osteoporosis group(25.00%vs 16.57%,P=0.026).Multivariate logistic regression analysis showed that female,BMI,and Hb levels were independent influencing factors for osteoporosis(OR=3.10,P<0.001;OR=0.86,P<0.001;OR=0.90,P=0.018,respectively).Conclusion Hb levels are significantly associated with the occurrence of osteoporosis in elder adults with T2DM.
