摘  要:目的探讨原发性高血压脉图参数与肾功能指标的相关性。方法应用ZBOX-Ⅰ型中医脉象仪采集152例原发性高血压病病人和43名健康人压力脉图时域特征参数,比较健康对照组、高血压胱抑素C正常组和高血压胱抑素C升高组脉象时域参数差异及肾功能指标(尿酸、尿素氮、肌酐、胱抑素C)与脉象参数(h1、h3、h4、h3/h1、t、as/ad等)的相关性。结果高血压胱抑素C正常组h3/h1、as/ad高于健康对照组(P<0.05),高血压胱抑素C升高组h1、h3、h4、h3/h1、as、ad、as/ad高于健康对照组(P<0.05),高血压胱抑素C正常组、高血压胱抑素C升高组t、t1、t4均较健康对照组延长(P<0.05)。高血压胱抑素C正常组较高血压胱抑素C升高组h1、h3、h4、as、ad明显增高(P<0.05)。原发性高血压脉象时域参数h1、t4、ad、h4/h1、w、as与尿素氮指标相关。结论高血压合并胱抑素C升高病人脉象时域参数变化特征可能与胱抑素C升高后引起主动脉及血管壁顺应性差、血管外周阻力升高、脉搏波传播速度快有关,脉象参数特征变化可一定程度反映尿素氮变化。Objective To analyze the correlation between electropulsogram parameters and renal function indexes in patients with essential hypertension.Methods The electropulsogram time domain characteristic parameters of 152 patients with essential hypertension and 43 healthy persons were collected by ZBOX-ⅠTCM pulse instrument.The difference of time domain characteristic parameters were compared between healthy control group,normal cystatin C of essential hypertension group and elevated cystatin C of essential hypertension group.The correlation between renal function parameters(uric acid,blood urea nitrogen,creatinine,cystatin C)and electropulsogram parameters(h1,h3,h4,h3/h1,t,as/ad)was analyzed.Results The results of h3/h1 and as/ad in normal cystatin C group were significantly higher than those in healthy control group(P<0.05).The results of h1,h3,h4,h3/h1,as and ad in normal cystatin C of essential hypertension group and elevated cystatin C of essential hypertension group were significantly longer than those in healthy control group.The h1,h3,h4,as and ad in normal cystatin C of essential hypertension group were significantly longer than those in elevated cystatin C of essential hypertension group(P<0.05).The correlation between the time domain parameters(including h1,t4,ad,h4/h1,w and as)and the blood urea nitrogen of essential hypertension was statistically significant.Conclusion The changes of temporal parameters in patients with essential hypertension might be related to the poor compliance of aorta and vessel wall,the increase of peripheral vascular resistance,and the rapid propagation of pulse wave,which were caused by the elevation of cystatin C.The variation of electropulsogram parameters might reflect the change of urea nitrogen in some extent.
