椎间盘微创术后患者子午流注择时艾灸联合耳穴埋籽应用效果观察

摘　　要：目的探讨子午流注择时艾灸联合耳穴埋籽法对腰椎间盘突出症(腰突病)微创术后患者临床应用效果。方法选取2017年01月~2020年10月间我院骨伤科56例腰椎间盘突出症微创术后患者,按照术后护理干预手段的不同,将其中28例术后接受腹部热敷、按摩及药物干预患者设为对照组,另外28例患者术后早期遵循子午流注法择时给予艾灸并联合耳穴埋籽法中医护理干预。术后1周,运用疼痛视觉模拟评分(VAS)、日本骨科协会(JOA)评分及健康量表评分(SF-36)作为评估工具,比较两组患者经不同干预后的临床疗效及治疗安全性。结果两组患者术后一周内尿潴留、腹胀、及腰/下肢疼痛评分(VAS)、症状评分(JOA)比较均有明显差异,其中观察组腹胀、尿潴留及腰/下肢疼痛发生率均明显低于对照组(P<0.05);观察组JOA评分明显高于对照组(P<0.05),观察组VAS评分明显低于对照组(P<0.05);观察组SF-36各维度评分中除情感职能和精神职能外,其余各维度得分均明显高于对照组(均P<0.05)。结论遵循子午流注择时艾灸联合耳穴埋籽法,可促进脊柱外科椎间盘微创术后患者气血运行,有效降低患者术后尿潴留及腹胀发生率,缓解腰/下肢疼痛症状,减轻患者痛苦,提高生活质量。Objective To investigate the clinical application effect of moxibustion combined with ear point seed embedding method on lumbar intervertebral disc herniation(lumbar protrusion disease)after minimally invasive surgery.Methods From January 2017 to October 2020,56 patients with minimally invasive lumbar disc herniation in the Department of Orthopedics and Traumatology of our hospital were selected.According to the different postoperative nursing interventions,28 of them received abdominal hot compress,massage and The drug intervention patients were set as the control group.The other 28 patients were given moxibustion at the appropriate time following the Ziwu Liuzhu method in the early postoperative period and combined with the traditional Chinese medicine nursing intervention of ear acupoint burying method.One week after surgery,the visual analogue scale for pain(VAS),the Japanese Orthopedic Association(JOA)score and the health scale score(SF-36)were used as evaluation tools to compare the clinical efficacy and treatment safety of the two groups of patients after different interventions.Results There were significant differences in urinary retention,abdominal distension,and waist/lower limb pain score(VAS)and symptom score(JOA)in the two groups within one week after surgery.The observation group had significant differences in the incidence of abdominal distension,urinary retention,and waist/lower limb pain.Lower than the control group(P<0.05);the JOA score of the observation group was significantly higher than that of the control group(P<0.05),and the VAS score of the observation group was significantly lower than the control group(P<0.05);the observation group SF-Except for the emotional function and mental function,the scores of the other dimensions of the 36 scores were significantly higher than those of the control group(all P<0.05).Conclusion Following the method of selective moxibustion and ear acupoint burying of Ziwuliu injection can promote the circulation of blood in patients after minimally invasive int
