摘　　要：目的初步探讨应用激光从睾丸不动精子中筛选活精子行卵细胞浆内单精子注射(ICSI)的结局。方法将一名因手淫取精失败行睾丸精子抽吸术(TESA)获得不动精子的患者女方卵子随机分为两组:实验组(14枚卵子)用激光法筛选活精子行ICSI,对照组(12枚卵子)用机械接触法直接行ICSI。比较两组的2PN受精率、优质胚胎率、优质囊胚形成率。结果实验组的2PN受精率高于对照组,但差异无统计学意义(92.9%vs 75.0%,P=0.208);实验组的优质胚胎率显著高于对照组(58.3%vs 11.1%,P=0.027);实验组的优质囊胚形成率高于对照组,但差异无统计学意义(27.3%vs 0%,P=0.107)。结论激光法能有效筛选存活的不动精子,可明显改善睾丸不动精子行ICSI的实验室结局。Objective Analyze the outcomes of ICSI using laser to select viable sperm from testicular immotile sperm.Methods The female eggs from a patient with immotile sperm obtained by testicular sperm aspiration due to the failure of masturbation for sperm extraction were randomly divided into two groups:the experimental group(14 eggs)performing ICSI by laser-assisted immotile sperm selection,and the control group(12 eggs)performing ICSI directly by mechanical contact method.2PN fertilization rate,high qualified embryo rate,and good blastocyst formation rate were compared between the two groups.Results The 2PN fertilization rate in the experimental group was higher than that in the control group,but the difference was not statistically significant(92.9%vs 75.0%,P=0.208).The high qualified embryo rate of the experimental group was significantly higher than that of the control group(58.3%vs 11.1%,P=0.027).The good blastocyst formation rate in the experimental group was higher than that in the control group,but the difference was not statistically significant(27.3%vs 0%,P=0.107).Conclusion The laser method can effectively select the viable immotile spermatozoa and significantly improve the laboratory outcome of ICSI by using testicular immotile sperm.
