摘  要:目的观察电针刺激百会穴、夹脊穴对脊髓损伤大鼠神经病理性疼痛的改善作用,并探讨其可能作用机制。方法45只雄性Wistar大鼠随机分为电针组、模型组及对照组,每组15只。电针组、模型组制备脊髓损伤模型,对照组正常饲养。电针组造模后第7天时对百会穴、脊柱两侧夹脊穴进行电针治疗,1次/天,共治疗7 d。分别于造模后第3、7、14天测量各组大鼠机械性触痛阈值(paw withdrawal pressure threshold,PWPT)。造模后第14天取各组大鼠脊髓,观察脊髓损伤部位形态变化;造模后第7、14天时抽取大鼠尾静脉静脉血,ELISA法检测各组大鼠血清白细胞介素(interleukin,IL)-1、IL-6、肿瘤坏死因子α(TNF-α)。结果与对照组比较,造模后第7、14天模型组PWPT降低(P<0.05),一直维持到术后14 d。与造模后第7天比较,造模后第14天电针组PWPT升高(P<0.05)。对照组脊髓的细胞形态学正常、组织结构完整;模型组可见脊髓组织结构破坏严重,可见脊髓空洞,神经元及尼氏体显著减少,细胞呈空泡样状,伴随细胞核的固缩;电针组可见脊髓组织的损伤程度减轻,神经元及尼氏体较前增多。与对照组比较,造模后第7、14天模型组大鼠血清IL-1、IL-6、TNF-α水平升高(P均<0.05);与造模后第7天比较,造模后第14天时电针组大鼠血清IL-1、IL-6、TNF-α水平下降(P均<0.05)。结论电针刺激百会穴、夹脊穴可改善脊髓损伤大鼠的神经病理性疼痛情况,改善镜下脊髓组织损伤,其机制可能为降低血清IL-1、IL-6及TNF-α水平。Objective To observe the improvement effect of electroacupuncture at Baihui and Jiaji points on the neuropathic pain in rats with spinal cord injury and to explore the possible mechanism.Methods Forty-five male Wister rats were randomly divided into the electroacupuncture group,model group and control group,with 15 rats in each group.In the electroacupuncture group and the model group,the spinal cord injury models were prepared,while the rats in the control group were fed normally.In the electroacupuncture group,electroacupuncture treatment was performed at Baihui point and Jiaji points on both sides of the spine on the 7th day of modeling,once per day,for 7 days.The paw withdrawal pressure thresholds(PWPT)of each group were measured on the 3rd,7th and 14th days.On the 14th day of modeling,the spinal cords of rats in each group were collected and morphological changes of spinal cord injury sites were observed.The venous blood of the tail vein was extracted on the 7th and 14th days of modeling,and the serum interlukin-1(IL-1),IL-6,and tumor necrosis factor(TNF)-αof rats in each group were detected by ELISA.Results Compared with the control group,PWPT was reduced in the model group on the 7th and 14th days(both P<0.05),and maintained until the 14th day after operation.Compared with the 7th day,PWPT in the electroacupuncture group increased on the 14th day(P<0.05).The cell morphology of spinal cord in the control group was normal and the tissue structure was intact.In the model group,the structure of the spinal cord tissues was seriously damaged,and the spinal cavity,neurons and nidellae were significantly reduced.The cells were vacuolated,accompanied by nuclear contraction.In the electroacupuncture group,the spinal cord injury was reduced,and the number of neurons and nissl increased.Compared with the control group,the serum levels of IL-1,IL-6 and TNF-αincreased in the model group on the 7th and 14th days(all P<0.05).The levels of IL-1,IL-6 and TNF-αin the electroacupuncture group decreased on the 14th day comp
