改良双环法与无垂直瘢痕下蒂瓣法乳房缩小术疗效比较

摘　　要：目的:对比分析改良双环法和无垂直瘢痕下蒂瓣法在乳房缩小整形术中的临床效果。方法:回顾总结乳房肥大女性患者30例的临床资料,按手术方式的不同将其分为A组和B组,均为15例,其中A组行改良双环法乳房缩小术,B组行无垂直瘢痕下蒂瓣法乳房缩小术,术后均随访6个月~1年。记录患者满意度主观评价、医师对术后乳房评价、术后并发症情况等。结果:两组患者症状均明显缓解,乳房体积明显减小,外观良好,术者医师评价与患者主观满意度之间基本一致,且组间比较差异无统计学意义(P>0.05)。A组乳头乳晕感觉及术后瘢痕情况均优于B组,差异有统计学意义(P<0.05)。结论:两种方法均可以得到较好的临床效果。但是适应证略有差别,轻中度乳房肥大伴下垂,且要求术后瘢痕隐蔽的患者宜选改良双环法缩小术,中重度乳房肥大伴下垂的患者宜选无垂直瘢痕下蒂瓣法乳房缩小术。Objective To compare the clinical effect of modified double ring method and inferior pedicle flap without vertical scar in reduction mammaplasty.Methods This retrospective study enrolled 30 female patients with breast hypertrophy.Patients were categorized according to types of operation,they were divided into the group A and the group B,each group 15 cases.Patients in the group A underwent modified double ring method,while patients in the group B underwent inferior pedicle flap without vertical scar.The follow-up period lasted 6 months to 1 year.Patient satisfaction,postoperative evaluation,and postoperative complications were assessed.Results Both treatments reduced breast volume significantly and relieved symptoms.Doctors’evaluation on surgery and patients'satisfaction were basically the same,there was no significant difference between the two groups(P>0.05).The nipple areola sensation and postoperative scar in the group A were better than those in the group B,the differences were statistically significant(P<0.05).Conclusion Both methods achieve better clinical outcomes,but the indications between two methods are slightly different.For patients with mild to moderate breast hypertrophy with ptosis,and the postoperative scar concealment,the modified double ring method should be selected.For patients with moderate or severe breast hypertrophy and ptosis,breast reduction with pedicle flap without vertical scar should be selected.
