自拟苦参汤坐浴联合湿疹膏外用治疗慢性肛周湿疹100例临床观察

摘　　要：目的观察自拟苦参汤坐浴联合湿疹膏外用治疗慢性肛周湿疹的临床疗效。方法将200例慢性肛周湿疹患者采用随机数字表法分为治疗组和对照组,各100例。治疗组予自拟苦参汤坐浴联合湿疹膏外用治疗,对照组予曲安奈德益康唑乳膏外用。治疗2周后比较2组的临床疗效、不良反应发生率及治疗前后症状评分。结果治疗组总有效率为87%(87/100),对照组为67%(67/100),2组比较差异有统计学意义(P<0.05);2组治疗后瘙痒评分、皮损评分及症状总评分均明显降低,与同组治疗前比较差异有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后比较差异有统计学意义(P<0.05);治疗组不良反应发生率(2%)明显低于对照组(10%),2组比较差异有统计学意义(P<0.05)。结论自拟苦参汤坐浴联合湿疹膏外用治疗慢性肛周湿疹疗效显著,能明显改善患者的瘙痒及皮损症状,且安全性高,值得临床推广应用。Objective To observe the clinical efficacy of sitz bath with self-made Kushen Tang(苦参汤)combined with external application of Shizhen Gao(湿疹膏)in the treatment of chronic perianal eczema.Methods Totally 200 cases of chronic perianal eczema were divided into treatment group and control group by random number table method,with 100 cases in each.The treatment group was given self-made Kushen Tang combined with external application of Shizhen Gao,while the control group was given external application of triamcinolone acetonide and econazole nitrate cream.After 2 weeks of treatment,the clinical efficacy,adverse reaction rate and symptom scores before and after treatment of the 2 groups were compared.Results The total effective rate of the treatment group was 87%(87/100),that of the control group was 67%(67/100),and the difference between the 2 groups was statistically significant(P<0.05).After treatment,the itching score,skin lesion score and total symptom score of the2 groups were significantly decreased,and the difference was statistically significant compared with the same group before treatment(P<0.05),moreover,the treatment group decreased more significantly,and the difference was statistically significant compared with the control group after treatment(P<0.05).The incidence of adverse reactions in the treatment group(2%)was significantly lower than that(10%)in the control group,and the difference was statistically significant between the 2 groups(P<0.05).Conclusion It has significant efficacy and can markedly improve the symptoms of itching and skin lesions with good safety to use sitz bath with self-made Kushen Tang combined with external application of Shizhen Gao in the treatment of chronic perianal eczema,which deserves clinical promotion and application.
