补肾止血汤结合间苯三酚注射液治疗先兆流产疗效及对PIBF水平的影响

摘　　要：目的评价补肾止血汤结合间苯三酚注射液治疗先兆流产的临床疗效及对PIBF水平的影响。方法选择2016年2月—2019年4月医院收治的先兆流产患者85例,依据随机对照法将患者分为观察组(43例)和对照组(42例)。两组患者确诊后要卧床静养,增加营养支持同时禁止性生活,口服叶酸、维生素E等。对照组在此基础上,采用间苯三酚注射液治疗。观察组在对照组治疗的基础上,口服补肾止血汤治疗。两组均治疗2周。比较两组治疗前后中医症候积分、治疗疗效,比较治疗前后性激素水平、PIBF水平,比较两组妊娠结局。结果观察组治疗总有效率为93.02%(40/43)高于对照组治疗总有效率的76.19%(32/43)(P<0.05);观察组治疗后阴道流血时间、阴道流血量、阴道流血色质、腰膝酸软和下腹坠胀等临床症状积分低于对照组(P<0.05);观察组治疗后性激素E2、P和β-HCG水平高于对照组(P<0.05);观察组治疗后PIBF水平高于对照组(P<0.05);观察组患者中足月妊娠率高于对照组,早产率低于对照组(P<0.05)。结论补肾活血汤结合间苯三酚治疗先兆流产疗效确切,可有效的改善患者的临床症状,提高性激素水平,改善胎儿生长环境,改善妊娠结局,保胎效果好。Objective To evaluate the clinical effect of Bushen Zhixue Decoction combined with phloroglucinol injection in the treatment of threatened abortion and its effect on PIBF level. Methods Eighty-five patients with threatened abortion admitted to our hospital from February 2016 to April 2019 were selected and divided into observation group(43 cases) and control group(42 cases) according to the randomized control method. Patients in the two groups were required to lie in bed for rest after diagnosis, increase nutrition support and forbid sex life, and take folic acid and vitamin E orally. On this basis, the control group was treated with phloroglucinol injection. The observation group was treated with Bushen Zhixue Decoction on the basis of the control group. Both groups were treated for two weeks. Before and after treatment, the scores of TCM symptoms, therapeutic effect, sexual hormone level, PIBF level and pregnancy outcome of the two groups were compared. Results The total effective rate in the observation group was 93.02%(40/43), which was higher than that(76.19%,32/42) in the control group(P<0.05). After treatment, the scores of clinical symptoms such as vaginal bleeding time, vaginal bleeding volume, vaginal bleeding quality, soreness of waist and knee, and swelling of lower abdomen in the observation group were lower than those in the control group(P<0.05). The levels of E2, P and β-HCG in the observation group were higher than those in the control group(P<0.05). The level of PIBF in the observation group was higher than that in the control group(P<0.05). The rate of full-term pregnancy in the observation group was higher than that in the control group, and the rate of premature delivery was lower than that in the control group(P<0.05). Conclusion Bushen Huoxue Decoction combined with phloroglucinol is effective in the treatment of threatened abortion. It can effectively improve the clinical symptoms of the patients, the levels of sex hormones, the growth environment of the fetus and the outcome of pregnancy
