雷火神针产生、发展及适应证探讨

摘　　要：考察明清时期的中医古籍文献及近现代文献,结合当时寒冷的自然环境及灸法发展的社会背景,梳理古代雷火神针疾病谱的变化及其在近现代的传承发展,探讨其适应证的改变。回顾中医古籍,发现与当时的自然及社会环境相适应,雷火神针适应证主要是病因病机为风寒湿侵袭、临床症状为疼痛的疾病,如鹤膝风、鸡脚风、脚气病等痹病,附骨疽及流注等外科病,杨梅疮及麻风等传染病。通过近现代文献梳理发现,雷火神针在演变过程中逐渐由实按灸变化为悬灸,药物组成及操作手法等亦发生改变,应用局限于原发性痛经、慢性胃炎、骨关节及软组织损伤等疾病,对于附骨疽、流注、杨梅疮及麻风等疾病应用较少。建议应临床进一步推广应用该治疗方法,并研究其起效机制。Based on the investigation of ancient Chinese medicine literature in the Ming and Qing Dynasties and modern Chinese medicine literature,combined with the cold natural environment and the social background of the development of moxibustion,the changes of disease spectrum of thunder-fire miraculous moxa stick and its inheritance and development in modern times were sorted out,and the changes of its indications were analyzed.Looking back on ancient Chinese medicine books,it was found that it was compatible with the natural and social environment at that time.The indications of thunder-fire miraculous moxa stick were mainly caused by wind,cold and dampness,and the clinical symptoms were painful diseases such as arthroncus of the knee and foot,spasm of hand and foot,beriberi,etc.,surgical diseases such as bone-attaching carbuncle and multiple abscess,and infectious diseases such as plum abscess and leprosy.Through the review of modern literature,it was found that it has gradually changed from actual pressing moxibustion to hanging moxibustion during the evolution process,and the composition of medicines and operation techniques have also changed,but its application is limited to primary dysmenorrhea,chronic gastritis,injury of bone joints and soft tissues.And other diseases,less used for diseases such as bone-attaching carbuncle,multiple abscess,plum abscess and leprosy.The method is worthy of further promotion and application in clinical practice,and studying its action mechanism.
