括约肌间瘘管结扎联合瘘管隧道式切除术治疗低位肛瘘的疗效分析

摘　　要：为探讨括约肌间瘘管结扎联合瘘管隧道式切除术治疗低位肛瘘的临床疗效,回顾2018年12月至2020年1月我院收治的86例低位肛瘘患者资料,其中瘘管切开术治疗49例(对照组),括约肌间瘘管结扎联合瘘管隧道式切除术治疗37例(观察组)。比较2组总体疗效、术后48h疼痛评分(VAS)、水肿程度、创面愈合时间、住院时间、术后3个月肛门功能。结果显示,观察组治愈率明显高于对照组,术后48hVAS评分明显低于对照组,肛缘水肿情况轻于对照组,创面愈合时间和住院时间均明显短于对照组,术后3个月肛门功能明显优于对照组,P<0.05。结果表明,与瘘管切开术相比,括约肌间瘘管结扎联合瘘管隧道式切除术治疗低位肛瘘患者术后疼痛、水肿更轻,创面愈合更快,住院时间更短,肛门功能损伤更小,效果更好。This study was to investigate the effect of low anal fistula treated ligation of intersphincteric fistula tract(LIFT)combined fistula-tract tunnelesque resection(FTTR),reviewed the data of 86 patients with low anal fistula treated in authors’hospital(2018-12-2020-01),including 49 cases(adopted fistulotomy)of control group and 37 cases(adopted LIFT plus FTTR)of observation group;then,compared both groups’whole effect(curative rate),score on pain at 48 hafter surgery(VAS),analedge edema intensity,wound-surface healing time,hospitalization time,and anal function at 3 months after surgery.As results,in above norms,observation group was respectively and significantly higher,lower,lighter,shorter,shorter than,and superior to control group(all,P<0.05).Results show that compared with fistulotomy,LIFT plus FTTR in the treatment of low fistula in ano has such advantages as more lighter postoperative pain and anal edge edema,more faster wound-surface healing,more shorter hospitaliation time,more less trauma to anal function,and more better efficacy.
