中药保留灌肠治疗溃疡性结肠炎最佳灌肠速度及插管深度的探讨

摘　　要：为探讨溃疡性结肠炎(UC)中药保留灌肠治疗时的最佳灌肠速度与插管深度,将我院收治的100例UC患者随机分为A、B、C、D 4组,均采用中药保留灌肠治疗,灌肠方相同,不同之处在于灌肠速度(ES)及插管深度(ID)。A组ES为100滴/min,ID为30cm;B组ES为60滴/min,ID为30cm;C组ES为100滴/min,ID为20cm;D组ES为60滴/min,ID为20cm。治疗8周后,比较4组总体疗效及治疗前后中医证候改善情况。结果显示,B组治疗总有效率明显高于其他3组(P<0.05),而其他3组间比较差异无统计学意义(P>0.05)。4组治疗前腹泻、腹痛、黏液脓血便、里急后重感评分及总分比较差异均无统计学意义(P>0.05);治疗后,4组各症状评分及总分均明显下降(P<0.05),各组间比较,B组各症状评分及总分均明显低于其他3组(P<0.05),其他3组间比较差异无统计学意义(P>0.05)。结果表明,中药保留灌肠治疗UC,ES为60滴/min、ID为30cm时效果最好。This study was to investigate the optimum enema speed(ES)and intubation depth(ID)about TCM retention enema in the treatment of ulcerative colitis(UC),randomly divided 100 UC patients treated in author’s hospital into four groups:A,B,C,D which all received a same enema formula,merely respectively with own ES and ID:in group A,ES=100 drops/min,ID=30 cm;in group B,60 drops/min,30 cm;in group C,100 drops/min,20 cm;in group D,60 drops/min,20 cm.After 8 weeks of treatment,their whole effect and TCM symptoms&signs improved from before treatment to after treatment were compared between the four groups.As results,in total effective rate there was no stastistical difference between group A,C,and D(P>0.05),only group B was significantly higher than group A,C,and D(P<0.05);before treatment in the score and totalled on diarrhea,abdominal pain,mucopurulent bloody stool,and tenesmus awareness there was no statistical difference between the four groups(P>0.05),but after treatment all groups’above score and totalled were declined significantly(P<0.05),merely group B’ones were significantly lower than rest 3 groups’ones(P<0.05),but between the 3 groups there was no statistical difference(P>0.05).Results show that in TCM retention enema for treating UC at 60 drops/min,and 30 cm,the most efficacy could be achieved.
