不同方式注射胰岛素控制结肠癌合并糖尿病患者围手术期血糖的效果比较

摘　　要：为比较皮下持续泵注与多次皮下注射胰岛素控制结肠癌合并糖尿病患者围手术期血糖的效果,选择2018年2月至2020年2月拟于我院接受手术治疗且合并糖尿病的82例结肠癌患者作为研究对象,随机分为观察组和对照组,各41例。2组患者均应用胰岛素控制血糖,但给药方式不同,观察组应用胰岛素泵予以皮下持续注射,对照组常规多次皮下注射。比较2组患者血糖控制情况、血糖达标时间、低血糖发生率及住院时间。结果显示,2组患者治疗前空腹血糖及餐后2h血糖比较差异均无统计学意义,P>0.05;治疗后,2组患者空腹血糖及餐后2h血糖均明显下降,达到目标血糖值,组间比较差异无统计学意义,P>0.05。观察组血糖达标时间、住院时间均明显短于对照组,低血糖发生率明显低于对照组,P<0.05。结果表明,皮下持续泵注与多次皮下注射胰岛素均可有效控制结肠癌合并糖尿病患者的围手术期血糖,但前者起效更快、安全性更高,临床推广价值更高。This study was to compare the efficacy of injecting insulin in way of hypodermic continuous pumping,and in way of multi-time hypodermic injection to control plasma glucose(PG)at perioperation of colonic cancer patients complicated with diabetes mellitus(DM),randomly divided 82 cases met above condition and all treated with insulin to control BG in author’s hospital(2018-02-2020-02)into observation group(41 cases,adopted the way of hypodermic continuous pumping)and control group(41 cases,the routine way of multi-time hypodermic injection);then,compared both groups’following indexes such as patients’PG controlled status,time for PG turned to objective value,hypoglycemia incidence,and hospitalization time.As results,in PG and postprandial 2-hrs PG before treatment there was no statistical difference between the two groups(P>0.05);but after treatment the two values were all declined significantly and down to objective one between the two groups there was no statistical difference(P>0.05),but in the time for down to objective value,and for hospitalization and in hypoglycemia incidence observation group was significantly shorter,and lower than control group(P<0.05).Results show that the two patterns of injecting insulin can all effectively control perioperative PG,but the way of hypodermic continous pumping is better with faster onset,higher safety,more worthy to be popularized clinically.
