某院人血白蛋白临床使用分析与评价

摘　　要：目的了解郑州市第二人民医院人血白蛋白的临床使用情况,为人血白蛋白的合理使用提供参考依据。方法利用医院HIS系统,调取郑州市第二人民医院2019年1月到12月涉及人血白蛋白的病历459份,根据人血白蛋白药品说明书相关指南及政策文件,对人血白蛋白的使用情况进行点评。结果459份病历中,男性267例(58.17%),女性192例(41.83%),患者年龄分布区间为6天到97岁之间,其中65岁以上282例(61.44%)、新生儿3例(0.65%)。白蛋白使用人数最多的科室为肿瘤科(114例),用药不合理例数最高的三个科室为肝胆外科(82.05%)、骨科(74.07%)和神经外科(62.05%),用药不合理原因排名靠前的有血清白蛋白浓度≥30g/L102例(55.14%)、用药指征不明确32例(17.30%)、营养支持21例(11.35%),另外未检测血清白蛋白浓度9例(4.86%),存在用药禁忌2例(1.08%)。结论通过临床药师的点评,本院人血白蛋白的使用仍存在很多不合理情况,应加强监管,点评标准也需要根据循证证据不断更新,临床药师审批和处方前置审核为实时监管人血白蛋白的合理使用提供了思路。Objective To understand the clinical use of human serum albumin in our hospital,and provide references for rational clinical application of human serum albumin.Methods Use the hospital HIS system to retrieve 459 medical records involving Human serum albumin in our hospital from January to December 2019.According to the Human serum albumin instructions,relevant guidelines and policy documents,the use of Human serum albumin Reviews.Results Among the 459 medical records,there were 267 males(58.17%)and 192 females(41.83%);The age distribution range of patients was between 6 days and 97 years old,282(61.44%)were over 65 years old and 3 were newborns(0.65%).The department with the largest number of albumin users was oncology(114 cases),and the three departments with the highest number of unreasonable medications were hepatobiliary surgery(82.05%),orthopedics(74.07%)and neurosurgery(62.05%).The top ranked causes of unreasonable medication were 102 cases(55.14%)with serum albumin concentration≥30 g/L,32 cases(17.30%)with unclear medication indications,21 cases(11.35%)with nutritional support,and serum albumin was not tested.The concentration was 9 cases(4.86%),and 2 cases(1.08%)were contraindicated.Conclusion According to the comments of clinical pharmacists,there are still many unreasonable situations in the use of Human serum albumin in our hospital.Supervision should be strengthened.The review standards should also be updated based on evidence-based evidence.Clinical pharmacist approval and pre-prescription review are real-time supervisors.The rational use of serum albumin provides ideas.
