摘　　要：目的总结方舱医院收治的轻型新型冠状病毒肺炎(COVID-19)患者临床特征以及方舱医院管理模式的探讨。方法纳入2020年2月17日至3月5日方舱医院收治的轻型COVID-19患者931例,对患者的基线资料、临床表现、疾病转归、实验室检查结果、影像学特点等进行统计学分析。结果轻型COVID-19年龄(46.68±12.13)岁,男女患病率差异无统计学意义;最常见的临床表现为发热(60.79%)、咳嗽(47.37%)、肌肉酸痛乏力(17.40%)、咽痛(13.86%)、咳痰(13.86%)、无症状(10.10%)、腹泻(9.98%)、胸闷呼吸困难(8.38%)、鼻塞(6.02%)、流涕(5.69%)等。另外精神焦虑、失眠占比12.13%。COVID-19分型:轻型248例(26.64%),普通型683(73.36%)。血生化检查中,少部分(26.32%)患者出现白细胞数降低;新型冠状病毒核酸检测结果大部分(55.45%)为阳性,阴性占44.55%;胸部CT检查阳性率94.36%,最多见的改变为毛玻璃影改变(48.70%)和多发小班片状影改变(44.58%)。转院患者65例(6.98%),转院的原因主要为COVID-19病情加重(32例,占49.2%)。结论方舱医院COVID-19患者总体病情比较轻,以普通型为主;需注意无症状阳性患者和无呼吸道症状的患者;CT仍为主要筛查手段;方舱医院可以承担轻型COVID-19患者的防疫救治。Objective To summarize the clinical characteristics of mild Coronavirus Disease 2019(COVID-19)patients and to discuss the management mode for the shelter hospitals.Methods Totally 931 patients with mild COVID-19 who were admitted to shelter hospitals were enrolled.The baseline data,main clinical manifestations,outcome,laboratory results and imaging results were statistically analyzed.Results The age of mild COVID-19 patients was(46.68±12.13)years,there was no significant difference of the prevalence rate between male and female.The most common clinical manifestations of mild COVID-19 patients were fever(60.79%),cough(47.37%),muscle soreness and weakness(17.40%),pharyngeal pain(13.86%),expectoration(13.86%),asymptomatic(10.10%),diarrhea(9.98%),chest distress and dyspnea(8.38%),nasal obstruction(6.02%),and runny nose(5.69%).In addition,mental anxiety and insomnia accounted for 12.13%.Among them,248 cases(26.64%)were mild type,and 683 cases(73.36%)were common type.The novel coronavirus nucleic acid test results showed that most of the patients were positive(55.45%),while negative accounted for 44.5%.The positive rate of chest CT examination was 94.36%.The most common changes were glass opacity(48.70%)and multiple small class(44.58%).COVID-19 The aggregation of the disease was the main cause(n=32,49.2%)of hospital transfer(n=65,6.98%).Conclusion The general condition of patients with COVID-19 in shelter hospitals is relatively mild,mainly of common type.Attention should be paid to asymptomatic positive patients and patients without respiratory symptoms.CT is still the main screening method.The shelter hospitals can undertake the rescue task with mild COVID-19 patients in epidemic prevention.
