大黄甘草汤加味坐浴联合芒硝外敷治疗重度混合痔术后肛缘水肿的效果观察

摘　　要：目的探究大黄甘草汤加味坐浴联合芒硝外敷在重度混合痔术后肛缘水肿患者中的应用效果。方法选取我院2017年6月~2019年6月收治的重度混合痔术后肛缘水肿患者68例,根据治疗方案不同分对照组和观察组各34例。对照组实施常规凡士林纱条换药,观察组给予大黄甘草汤加味坐浴联合芒硝外敷治疗。对比两组疗效、术后愈合时间及治疗前后水肿积分、视觉模拟评分法(VAS)评分,观察比较两组血清炎性因子:肿瘤坏死因子α(TNF-α)、白介素6(IL-6)水平。结果观察组治疗总有效率为94.12%,高于对照组的73.53%,差异有统计学意义(P<0.05);治疗后,观察组水肿积分、VAS评分均低于对照组,差异均有统计学意义(P<0.05);观察组术后愈合时间较对照组短,差异有统计学意义(P<0.05);治疗后观察组血清TNF-α、IL-6水平均低于对照组,差异均有统计学意义(P<0.05)。结论大黄甘草汤加味坐浴联合芒硝外敷治疗重度混合痔术后肛缘水肿患者,可降低血清炎性因子水平,促进症状改善、术后愈合,缓解疼痛,疗效显著。Objective To explore the effect of DahuangGancao Decoction plus sitting bath combined withmirabilite external application in patients with anal edema after severe mixed hemorrhoidsoperative.Methods 68 patients with anal edema after severe mixed hemorrhoids from June 2017 to June 2019 in our hospital were enrolled.According to different treatment regiments,34 patients were divided into control group and observation group respectively.The control group was given regular Vaseline gauze dressing change,and the observation group was given DahuangGancao Decoction plus sitting bath combined with mirabilite external application treatment.The efficacy of the two groups,postoperative healing time and edema score before and after treatment,visual analog scale(VAS)score,serum inflammatory factors levels were compared:tumor necrosis factor alpha(TNF-α)、interleukin-6(IL-6).Results The total effective rate was 94.12%in the observation group,higher than 73.53%in the control group,the difference was statistically significant(P<0.05).After treatment,edema score and VAS score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The postoperative healing time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the serum levels of TNF-and IL-6 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion DahuangGancao Decoction combined withmirabiliteexternal application for the treatment of patients with anal edema after severemixedhemorrhoids operative can reduce serum inflammatory factor levels,promote symptom improvement,postoperative healing,relieve painand have significant curative effect.
