复元活血汤对血流动力学不稳定性胫腓骨骨折早期愈合的临床研究

摘　　要：目的探讨复元活血汤对血流动力学不稳定性胫腓骨骨折早期愈合的临床研究。方法选择2017年6月~2019年11月我院骨科收治的胫腓骨骨折患者88例作为研究对象,依据随机非盲法分组,对照组(n=44)行常规处理,观察组(n=44)在对照组基础上结合复元活血汤干预,比较两组干预前后的消肿结局、VEGF、BMP-2、TNF-α,同时比较两组患者干预前后的凝血指标(PT、APTT、FIB)。结果与干预前比较,观察组及对照组的消肿结局、VEGF、BMP-2、TNF-α及凝血指标均得到明显改善(P<0.05);观察组干预后3 d及7 d的骨折消肿程度(2.01±0.59;1.11±0.27)明显优于对照组(2.51±0.79;1.76±0.42),差异有统计学意义(t=3.849,P<0.05);与对照组比较,接受干预后观察组VEGF、BMP-2等明显高于对照组,且其TNF-α值明显较低于对照组,差异有统计学意义(P<0.05);两组患者接受干预后15 d后,两组PT、APTT比较,差异无统计学意义(P>0.05),观察组的FIB水平明显低于对照组(2.21±0.52 vs.2.76±0.62,P<0.05)。结论以复元活血汤可明显提升胫腓骨骨折患者VEGF及BMP-2水平,降低其炎症发生风险及术后肿胀,促进患者血液高凝状态改善,有效稳定机体血流动力学,对骨折早期愈合的改善有积极的促进作用。Objective To investigate the clinical study of Fuyuan Huoxue Decoction on the early healing of hemodynamically unstable tibia and fibula fractures.Methods A total of 88 patients with tibia and fibula fractures who were admitted to our hospital from June 2017 to November 2019 were selected as the study subjects.According to random non-blinded grouping,the control group(n=44)was given routine treatment,and the observation group(n=44)was further given Fuyuan Huoxue Decoction on the basis of the control group.The swelling outcome,VEGF,BMP-2,TNF-αand other indicators were compared and observed in the observation group and the control group before and after intervention.At the same time,the coagulation indices(PT,APTT,FIB)in both groups before and after intervention were compared.Results Compared with those before intervention,the swelling outcome,VEGF,BMP-2,TNF-αand coagulation indices in the observation group and the control group were significantly improved(P<0.05);the degree of fracture anti-swelling(2.01±0.59;1.11±0.27)in the observation group was significantly better than that in the control group(2.51±0.79;1.76±0.42)at 3 d and 7 d after intervention,and the data comparison was statistically significant(t=3.849,P<0.05);compared with the control group,the VEGF and BMP-2 indices in the observation group were significantly higher than those in the control group after the intervention,and its TNF-αvalue was significantly lower than that in the control group.The differences were statistically significant(P<0.05);after 15 days of intervention,there were no differences in PT and APTT between the observation group and the control group(P>0.05).The FIB level in the observation group was significantly lower than that in the control group(2.21±0.52 vs.2.76±0.62,P<0.05).Conclusion Fuyuan Huoxue Decoction can significantly increase the levels of VEGF and BMP-2 in patients with tibia and fibula fractures reduce the risk of inflammation and postoperative swelling,promote the improvement of the patient's blood hypercoag
