摘  要:目的:探讨CT血管造影(CT angiography,CTA)低剂量扫描联合迭代重建在结肠检查中的可行性。方法:收集某院2018年1月至7月经结肠镜证实为结肠癌且行CT检查的42例患者(19 kg/m^2≤体质量指数<24 kg/m^2)临床及影像资料。动脉期行CTA低剂量扫描:管电压100 k V、管电流80 m As,采用i Dose43级迭代算法重建图像;静脉期行常规剂量扫描:管电压120 kV、管电流200 mAs,采用滤波反投影(filtered back projection,FBP)算法重建图像。对比静脉期和动脉期的图像质量及有效剂量(effective dose,ED),并以病理分期为金标准,评价CT分期的准确性。采用SPSS24.0软件进行数据分析。结果:动脉期与静脉期图像质量比较差异无统计学意义(P>0.05);动脉期ED显著低于静脉期ED(P<0.05),且动脉期ED比静脉期降低约70%。与病理分期比较,CT分期诊断准确率达85%以上。结论:CT血管造影低剂量扫描联合迭代重建能更清晰显示肿瘤血供及微血管等信息,进一步提高了CT分期诊断准确率,并在保证图像质量的前提下大幅降低患者所受的辐射剂量,在结肠检查中具有重要的应用价值。Objective To explore the feasibility of low-dose CT angiography(CTA)combined with iterative reconstruction(IR)during colon examination.Methods Clinical and imaging data of 42 colon cancer patients confirmed by colonoscopy and undergoing CT examination were collected from January to July 2018 at some hospital,who had the body mass indexes not lower than 19 kg/m^2 while less than 24 kg/m^2.At arterial phase low-dose CTA was executed with the tube voltage of 100 kV and the tube current of 80 mAs,and the image reconstruction was realized with i Dose43 iterative reconstruction algorithm;at venous phase conventional-dose scan was performed with the tube voltage of 120 k V and the tube current of 200 m As,and the image reconstruction was implemented with filtered back projection(FBP)algorithm.The images at arterial phase had the image quality and effective dose(ED)compared with those at venous phase,and the accuracy of CT staging was evaluated using pathological staging as the gold standard.SPSS 24.0 software was used for data analysis.Results The images at arterial phase had no significant differences with those at venous phase(P>0.05);the ED at arterial phase was statistically lower than that at venous phase(P<0.05),which was reduced by about 70%than the latter.The diagnosis accuracy by CT staging was higher than 85%when compared with pathological staging.Conclusion Low-dose CT angiography combined with iterative reconstruction clearly displays tumor blood supply and microvasculature and etc,further improving the diagnostic accuracy of CT staging and significantly reducing the radiation dose to patients while ensuring image quality,which is of great value in colon examination.[Chinese Medical Equipment Journal,2021,42(1):53-56]
