人体脊柱后凸矫形力的测试分析

摘  要:临床对于矫正轻度脊柱后凸的非手术方法大多是佩戴脊柱后凸矫形器,其通过对脊柱后凸起施加垂直于脊柱的向前的矫形力,该力由矫形器的肩带作用于肩部而产生。采用摄影的方法对脊柱后凸矫形过程中人体颈椎和胸椎段的标志点运动学数据进行记录;通过试验机提供垂直于脊柱沿矢状面方向向后的矫形拉力并记录;利用Image J图像处理软件对采集的图像序列进行非线性拟合分析,量化脊柱后凸的有效矫形力与矫形角度的对应关系;最终得到考虑性别、身高、身体质量指数、上半身长/身高、外肩宽、坐高、坐肩高/坐高等因素在内的矫形角度与对应的矫形力计算的通用公式。结果表明:矫形力与矫形角度函数关系可用三次函数表示,回归系数R2均大于0.99;将身高、体重等个体化因素数据考虑在内的拟合回归方程的P值均小于0.05,拟合方程显著性良好;且拟合的矫形通用公式可适用于根据身高、体重等尺度结合矫形度数初步计算所需矫形力大小。Most of the clinical non-surgical methods for the correction of mild kyphosis are to wear kyphosis orthosis,to apply a forward force perpendicular to the spine to the kyphosis,which is generated by the shoulder belt of the orthosis acting on the shoulder.The research on the relationship between orthopedic force and orthopedic angle has not been reported yet,and the relationship between them is of great significance for the design and optimization of orthopedics.In this paper,the method of photography was used to record the kinematic data of the mark points of the cervical vertebra and the thoracic vertebrae in the process of kyphosis orthopedic;the orthopedic pull force perpendicular to the spine along the sagittal direction was provided and recorded by Instron 5544;J image processing software was used to conduct non-linear fitting analysis on the collected image sequence and quantify the corresponding relationship between the effective orthopedic force and the orthopedic angle of kyphosis,and finally a general formula was established for calculating the orthopedic angle and corresponding orthopedic force by taking into account the factors of gender,height,body mass index,upper body length/height,outer shoulder width,sitting height,and sitting shoulder height/sitting height.The results show that the relationship between the orthopedic force and orthopedic angle can be expressed as a cubic function,and the regression coefficient R2 is greater than 0.99;the P value of the fitting regression equation,which takes into account the data of individual factors such as height and weight,is less than 0.05,and the significance of the fitting equation is good;and the general formula of the fitting orthopedic can be applied to the initial calculation of required shape correction force according to height,weight,and other dimensions combined with the orthopedic degree.
