摘　　要：目的探讨经蛛网膜下腔阻滞中使用不同麻醉药物的分娩镇痛效果。方法选择2016年5月至2019年10月我院经阴道分娩的60例产妇作为研究对象,根据摸球法将其随机分为观察组和对照组,每组30例。观察组予以布比卡因、舒芬太尼和右美托咪定麻醉,对照组予以布比卡因和舒芬太尼麻醉,比较两组的应用效果。结果镇痛后30、60 min,观察组的VAS评分均低于对照组(P<0.05);观察组的镇痛维持时间长于对照组(P<0.05)。观察组的总产程、宫口全开时间均短于对照组(P<0.05);两组胎儿娩出后1、5 min Apgar评分无显著差异(P>0.05)。观察组的并发症总发生率低于对照组(P<0.05);观察组术后SAS评分低于对照组(P<0.05)。结论经蛛网膜下腔阻滞中使用布比卡因复合舒芬太尼和右美托咪定的分娩镇痛效果好,不会对胎儿造成影响,临床医师可根据实际情况进行选择。Objective To explore the labor analgesia effect of different anesthetics in subarachnoid block.Methods A total of 60 cases of puerpera with vaginal delivery in our hospital from May 2016 to October 2019 were selected as the study objects and randomly divided into observation group and control group according to ball touching method,with 30 cases in each group.The observation group was given bupivacaine,sufentanil and dexmedetomidine anesthesia,while the control group was given bupivacaine and sufentanil anesthesia.The application effects of the two groups were compared.Results At 30 and 60 minutes after analgesia,the VAS scores of the observation group were lower than those of the control group(P<0.05);the duration of analgesia in the observation group was longer than that in the control group(P<0.05).The total labor and full opening time of the uterus in the observation group were shorter than those in the control group(P<0.05);there were no significant differences in Apgar scores of the fetus at 1 and 5 minutes after delivery between the two groups(P>0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05);the postoperative SAS score of the observation group was lower than that of the control group(P<0.05).Conclusion Bupivacaine combined with sufentanil and dexmedetomidine in subarachnoid block is effective in labor analgesia,and it will not affect the fetus.Clinicians can choose according to the actual situation.
