等离子激光术和小切口吸刮术治疗腋臭的疗效及安全性比较

摘　　要：目的比较等离子激光术和小切口吸刮术治疗腋臭的疗效及安全性。方法2018年1月-2019年1月第四军医大学西京医院皮肤科纳入68例腋臭患者,将患者随机平均分为两组,实验组34例,采用等离子激光术治疗,对照组34例,采用小切口吸刮术治疗。持续随访12个月,比较两组患者的治疗效果和并发症发生情况。结果实验组患者治疗有效率为94.12%(32/34),复发率为6.25%(2/32);对照组患者治疗有效率为97.06%(33/34),复发率为3.03%(1/33)。两组组间治疗有效率和复发率差异无统计学意义(P=0.555)。患者术后并发症包括皮下血肿和手术区域皮肤坏死,其中实验组术后无并发症发生,对照组并发症发生率为11.76%(4/34,1例皮下血肿,3例皮肤坏死)。两组间并发症发生率差异有统计学意义(P=0.039)。结论等离子激光术和小切口吸刮术治疗腋臭均有良好的疗效。与小切口吸刮术相比,等离子激光术治疗的并发症发生率更低,安全性更好。Objective To compare the clinical efficacy and safety of plasma laser therapy versus suction-curettage through small incision in the treatment of axillary osmidrosis.Methods We recruited 68 patients with axillary osmidrosis from Department of Dermatology,Xijing Hospital,Fourth Military Medical University from January 2018 to January 2019.Patients were randomly divided into two groups:one group was treated with plasma laser(experimental group,n=34),the other group was treated with small-incision suction and scraping(control group,n=34).After the follow-up of 12 months,clinical efficacy and complications were compared between these two groups.Results After one year's follow-up,the treatment effective rate of the experimental group was 94.12%(32/34),and the recurrence rate was 6.25%(2/32);the treatment effective rate of control group was 97.06%(33/34),and the recurrence rate was 3.03%(1/33).There was no statistically significant difference in treatment response rate and relapse rate between the two groups(P=0.555).Postoperative complications included subcutaneous hematomas and skin necrosis in the surgical area.The incidence of postoperative complications in the experimental group was 0,and the incidence of complications in the control group was 8.82%(4/34,1 case of subcutaneous hematoma,3 cases of skin necrosis).The incidence of complications was statistically different between the two groups(P=0.039).Conclusion In comparison to suction-curettage therapy,plasma laser therapy is proved to have equivalent effect and less postoperative complications.
