摘　　要：【目的】探讨促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)在弥漫性甲状腺肿伴甲状腺功能亢进(Graves病)患者血清中的表达水平及其意义。【方法】选取2017年1月至2018年6月在本院诊治的Graves病患者130例(观察组),另外选取同期在本院健康体检志愿者100例作为对照组,检测所有受试者血清TSH、TPOAb和TGAb水平,分析其临床意义。【结果】观察组TSH水平明显低于对照组,而TPOAb和TGAb水平明显高于对照组,其差异均有统计学意义(P<0.05);观察组伴和不伴眼突患者血清TSH、TPOAb、TGAb水平比较差异无统计学意义(P>0.05)。观察组治疗后6个月发生甲状腺功能减退(简称为甲减)患者33例;早发甲减患者治疗前TPOAb和TGAb水平明显高于未发生甲减患者(P<0.05)。【结论】Graves病患者血清TSH、TPOAb和TGAb水平明显异常,其中治疗前TPOAb和TGAb水平可能与患者治疗后甲减发生有关,TSH、TPOAb和TGAb水平与患者是否存在眼突无明显关系。【Objective】To investigate the expression level and significance of thyrotropin(TSH),thyroid peroxidase antibody(TPOAb)and thyroglobulin antibody(TGAb)in serum of patients with diffuse goiter with hyperthyroidism Graves Disease.【Methods】A total of 130 Graves patients(observation group)treated in our hospital from January 2017 to June 2018 were selected,and 100 healthy volunteers were selected as control group,serum TSH,TPOAb and TGAb levels were measured.【Results】The level of TSH in the observation group was significantly lower than that in the control group,while the level of TPOAb and TGAb in the observation group was significantly higher than that in the control group,and the differences were statistically significant(P<0.05);the level of TSH,TPOAb and TGAb in the observation group with and without exophthalmos had no statistically significant difference(P>0.05).In the observation group,there were 33 patients with hypothyroidism 6 months after treatment;the levels of TPOAb and TGAb in patients with early-onset hypothyroidism before treatment were significantly higher than those in patients without hypothyroidism(P<0.05).【Conclusion】The serum levels of TSH,TPOAb and TGAb were significantly abnormal in patients with Graves'disease.The levels of TPOAb and TGAb before treatment may be related to the occurrence of hypothyroidism after treatment,but the levels of TSH,TPOAb and TGAb have no significant relationship with the presence of exophthalmos.
