中医药治疗新型冠状病毒肺炎的经验与策略——张伯礼院士武汉一线抗疫思考

摘　　要：总结张伯礼院士治疗新型冠状病毒肺炎(简称“新冠肺炎”)的思路,提炼防控和救治经验。认为新冠肺炎属于中医学“湿毒疫”范畴,主要证候要素为湿、毒、热、闭、虚,主要侵袭肺与脾两脏;湿毒壅肺为主要病机,湿邪致病临床证治更加复杂。临证全程须注重祛湿化浊、消秽解毒,还须重视伴有血瘀状态的影响。在治疗上,辨证论治与群体通治相结合,急性疫病爆发时,在辨证的基础上宜采取群体通治方为主的治疗策略,可采取专病专方;在治疗节点上,宜关口前移,注重早期干预,明晰新冠肺炎演变趋势,提前用药截断逆转病势;在新型冠状病毒核酸检测转阴的康复阶段,可采用益气养阴等中药扶正,兼顾祛邪,同时采用中西医结合康复手段综合调摄形神,促进全面康复。Academician ZHANG Boli's treatment ideas for the treatment of coronavirus disease 2019(COVID-19)is summarized,and the prevention,control and treatment experience are refined.It is believed that the COVID-19 belongs to the category of“damp toxin epidemic”in traditional Chinese medicine(TCM).The main syndrome elements are dampness,toxin,heat,blockade,and deficiency,which mainly invade the lung and spleen;the main pathogenic mechanism is the damp and toxin obstructing lung,and the clinical diagnosis and treatment of diseases caused by dampness pathogen is more complicated.In the whole clinical process,attention must be paid to eliminating dampness and resolving turbidity,eliminating filth and removing toxicity,and to the effects of blood stasis.In terms of treatment,individualized syndrome differentiation and treatment are combined with group treatment.In the case of an acute disease outbreak,the treatment strategy based on group treatment should be adopted on the basis of syndrome differentiation and special prescriptions.Treatment should be taken as early as possible.We should pay attention to early intervention,clarify the evolution trend of COVID-19,use drugs in advance to reverse the disease;in the recovery stage when the novel coronavirus nucleic acid test turns negative,Chinese medicines of tonifying qi and replenishing yin can be used to strengthen the vital qi,and we should take into account the elimination of pathogens,and use integrated traditional Chinese and western medicine rehabilitation methods to comprehensively adjust body and spirit and to promote comprehensive recovery.
