咸阳市第一人民医院475例碳青霉烯类抗菌药物使用合理性评价及经济学分析

摘　　要：目的分析咸阳市第一人民医院近3年出院患者碳青霉烯类抗菌药物应用情况,为该类抗菌药物的管理及临床合理应用提供参考。方法回顾性分析咸阳市第一人民医院2017年1月1日—2019年12月31日出院的475例应用碳青霉烯类抗菌药物患者的病案资料,对患者的基本信息、用药科室、患者感染部位、用法用量、送检情况以及适应症等运用WPS 2020进行统计分析。结果用药数量:美罗培南>亚胺培南-西司他丁>比阿培南。用药科室分布:呼吸与危重症医学科第一、中心ICU第二,其余科室相对较少。药物用法:静脉滴注473例,超声透药2例。药物用量:91例剂量不足,47例剂量过高。联合用药情况:与抗菌药物联用195例,与丙戊酸钠联用6例。病原学送检率88.84%。适应症:48例为无适应症用药。结论咸阳市第一人民医院碳青霉烯类抗菌药物临床应用合理性有待提高,合理应用抗菌药物可节约部分医疗花费。Objective To analyze the application of carbapenem antibiotics in the First People's Hospital of Xianyang in recent three years,to provide reference for the management and clinical rational application of carbapenems antibiotics.Methods The medical records of 475 patients with carbapenem antibiotics who were discharged from the First People's Hospital of Xianyang from 1st January 2018 to 31th December 2019 were retrospectively analyzed.WPS 2020 was used to analyze the basic information,medication department,infection site,usage and dosage,submission and indications of patients.Results Quantity:Meropenem>imipenem cilastatin>biapenem.Department distribution:the first is Department of Respiratory and Critical Care Medicine,and second is ICU,other departments are relatively few.Drug usage:473 cases patients were administrated by intravenous drip,and 2 cases patients were administrated by transdermal ultrasound.Drug dosage:91 cases were gived with insufficient dose drug,and 47 cases were gived with over dose drug.Combination of drugs:195 cases were treated with antibiotics,and 6 cases were treated with sodium valproate.Etiological examination rate:88.84%.Indications:48 cases were judged to have no indication.Conclusion The rationality of clinical application of carbapenem antibiotics in the First People's Hospital of Xianyang needs to be improved,and rational use of antibiotics antibiotics can save part of the medical expenses.
