摘　　要：目的了解甘肃省甘南藏族自治州(简称甘南州)居民死因顺位及潜在减寿年数(PYLL)构成特征,为制定重大疾病防制策略提供科学依据。方法整理甘南州各级医疗机构收集的死因数据,按照ICD-10标准开展死因分类,采用EXCEL 2013和SPSS 19.0分类汇总并分析数据,分析指标为粗死亡率、标化死亡率、潜在减寿年数和平均减寿年数(AYLL)。结果2019年甘南州居民粗死亡率为569.15/10万、标化死亡率449.76/10万;报告死亡前四位的疾病为循环系统疾病(粗死亡率201.70/10万、标化死亡率156.81/10万)、呼吸系统疾病(粗死亡率138.31/10万、标化死亡率106.14/10万)、肿瘤(粗死亡率115.22/10万、标化死亡率89.09/10万)、损伤和中毒(粗死亡率43.56/10万、标化死亡率40.43/10万);4类疾病报告死亡占全死因病例的87.64%;全死因潜在减寿年数顺位第一位为损伤和中毒(4646人年),损伤中毒类潜在减寿年数顺位第一位为机动车辆交通事故(2024人年);全死因平均减寿年数第一位为先天畸形、变形和染色体异常(54年/人),损伤和中毒类平均减寿年数第一位为意外窒息(49年/人)。结论影响甘南州居民健康的主要死因是循环系统疾病和呼吸系统疾病,损伤和中毒及先天异常对当地造成的疾病负担较重;应针对这些疾病提出行之有效的防控措施。Objective To understand the constitutive features of the rank order of death causes and the potential years of life lost(PYLL)in permanent residents of Gannan Tibetan autonomous prefecture(Gannan prefecture)of Gansu province in 2019,so as to provide scientific basis for formulating prevention and control strategies of critical illness.Methods The data of death causes collected from all medical institutions at all levels in Gannan prefecture were sorted out,and the causes of death were classified according to the standard of ICD-10.The data were summarized and statistically analyzed by EXCEL2013 and SPSS 19.0 software,with the analysis indexes of crude death rate,standardized mortality rate,PYLL and average years of life lost(AYLL).Results The crude death rate was 569.15/100000,with the standardized mortality rate of 449.76/100000 in the residents of Gannan prefecture in 2019.The top four diseases causing death reported were circulation system diseases(crude death rate of 201.70/100000,standardized mortality rate of 156.81/100000),respiratory diseases(crude death rate of 138.31/100000,standardized mortality rate of 106.14/100000),tumors(crude death rate of115.22/100000,standardized mortality rate of 89.09/100000),injuries and poisoning(crude death rate of 43.56/100000,standardized mortality rate of 40.43/100000).The reported deaths caused by the four kinds of diseases accounted for 87.64%of all deaths.As the PYLL rank order in all-causes mortality,the first one was injuries and poisoning(4646 person years),and traffic accident(2024 person years)ranked the first of injuries and poisoning.As the AYLL rank order of all-causes mortality,the first one was congenital malformations,deformations and chromosomal abnormalities(54 years/persons),and sudden asphyxia(49 years/persons)was at the first place in injuries and poisoning.Conclusions The main causes of death are circulation system and respiratory system diseases in residents in Gannan prefecture,2019,and the burden of disease caused by injury poisoning and congenital
