超声引导下水压灌肠对急性肠套叠患儿应激反应及胃肠激素的影响

摘　　要：目的探讨超声引导下水压灌肠对急性肠套叠(acute intussusception,AI)患儿应激反应及胃肠激素的影响。方法选择2018年3月至2019年8月我院收治的AI患儿148例,采用随机数字表法分为超声组76例和X射线组72例。超声组采用超声引导下水压灌肠治疗,对照组采用X射线引导下空气灌肠治疗,比较两组患儿治疗效果、应激反应、胃肠激素及并发症。结果超声组AI患儿复位成功率98.68%高于X射线组91.67%,复发率和并发症低于X射线组(P<0.05),复位时间、大便隐血恢复时间、住院时间短于X射线组(P<0.01);血清白细胞介素-6(IL-6)、C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)含量低于X射线组(P<0.01);血清胃动素(MTL)、胃泌素(GAS)、神经降压素(NT)含量低于X射线组(P<0.01)。结论超声引导下水压灌肠有助于缓解AI患儿炎症应激反应,降低胃肠激素水平,提高治疗效果,减少并发症的发生。Objective To investigate the stress response of water pressure enema under the guidance of ultrasound for children with acute intussusception(AI).Methods One hundred and forty-eight children with AI in our hospitals from March 2018 to August 2019 were divided into ultrasonic group(n=76)and X-ray group(n=72).The ultrasound group given ultrasound-guided water enema while the control group given X-ray guided air enema.The therapy effect,stress reaction,gastrointestinal hormones and complications were compared between the two groups.Results The reduction successful rate in the ultrasound group was 98.68%which was higher than that in the X-ray group(91.67%).The recurrence rate and complications in the ultrasound group was lower than that in the X-ray group(P<0.05).The reduction time,fecal occult blood recovery time,and hospital stay time in the ultrasound group were shorter than that in the X-ray group(P<0.01).The levels of serum IL-6,CRP,TNF-α,MTL,GAS and NT in the ultrasound group were lower than those in the X-ray group(P<0.01).Conclusion The water pressure enema under the guidance of ultrasound helps to alleviate the inflammatory stress response,reduce the gastrointestinal hormone levels,improve the therapy effect and reduce the complications.
