子午流注针法配合辨证取穴治疗肝火上扰型耳鸣的临床疗效观察

摘　　要：目的:观察子午流注针法配合辨证取穴治疗肝火上扰型耳鸣的临床疗效。方法:将符合纳入标准的81例耳鸣患者按随机数字表分为治疗组、对照组A和对照组B,每组27例。对照组A采用传统针刺进行治疗,对照组B给予每日3次口服弥可保0.5 mg,治疗组采用子午流注针法配合辨证取穴,疗程为2周。观察3组治疗前,治疗1个疗程后耳鸣评价量表(TEQ)评分,中医证候评分的变化,并比较3组的临床疗效。结果:治疗1个疗程后,3组耳鸣评价量表(TEQ)评分,中医证候评分较同组治疗前均下降(P<0.05),治疗组耳鸣评价量表(TEQ)评分低于对照组A和对照组B(P<0.05),对照组A和对照组B耳鸣评价量表(TEQ)评分比较差异无统计学意义(P>0.05);治疗组,对照组A较对照组B可以显著降低中医证候评分(P<0.01),治疗组中医证候评分同对照组A比较,差异无统计学意义(P>0.05)。治疗组,对照组A,对照组B临床疗效总有效率分别为92.59%(25/27),77.78%(21/27),74.07%(20/27),治疗组总有效率优于对照组A和对照组B(P<0.05)。结论:子午流注针法配合辨证取穴治疗肝火上扰型耳鸣疗效优于传统针刺、口服弥可保,能有效的缓解症状,提高痊愈率。Objective:To observe the clinical efficacy of midnight-midday ebb flow acupuncture combined with points selection based on syndrome differentiation in treating tinnitus of flaring up of Liver-fire.Methods:81 patients with tinnitus who met the inclusion criteria were randomly divided into the treatment group,the control group A and the control group B,with 27 cases in each group.The control group A was intervened by traditional acupuncture,the control group B was given orally Mecobalamin(0.5 mg,three times a day)and the treatment group was treated with midnight-midday ebb flow acupuncture combined with points selection based on syndrome differentiation.The three groups were treated for two weeks.The scores of the Tinnitus Evaluation Questionnaire(TEQ)and TCM syndrome were observed before the treatment and after one course of the treatment in the three groups.The clinical efficacy was compared among the groups.Results:After one course of the treatment,the scores of TEQ and TCM syndrome were significantly decreased in the three groups(P<0.05);the decreases in TEQ scores were more significant in the treatment group than those in the other two groups(P<0.05);there were no statistical difference in the scores of TEQ between the control group A and the control group B(P>0.05);the TCM syndrome scores were lower in the treatment group and the control group A than those in the control group B(P<0.01),but there were no difference in TCM syndrome scores between the treatment group and the control group A(P>0.05).The total effective rate was 92.59%(25/27)in the treatment group,which was significantly higher than 77.78%(21/27)and 74.07%(20/27)in the control group A and the control group B(P<0.05).Conclusion:The midnight-midday ebb flow acupuncture combined with points selection based on syndrome differentiation can effectively relieve symptoms and improve the cure rate in the treatment of tinnitus of flaring up of Liver-fire,which is better than conventional acupuncture.
