体表定位丘脑血肿引流术联合侧脑室穿刺引流术治疗丘脑出血破入脑室患者的临床研究

摘　　要：目的:研究体表定位丘脑血肿引流术联合侧脑室穿刺引流术治疗丘脑出血破入脑室患者的临床效果。方法:选取2017年4月-2019年4月濮阳市人民医院丘脑出血破入脑室患者92例,按手术方案不同分为联合组(n=46)、参照组(n=46)。参照组采用侧脑室穿刺引流术治疗,联合组在参照组基础上联合采用体表定位丘脑血肿引流术治疗。比较两组脑室引流管带管时间、颅内感染发生率、治疗前及治疗后1个月血清转铁蛋白水平。结果:联合组脑室引流管带管时间短于参照组(P<0.05);联合组颅内感染发生率为4.35%,低于参照组的17.39%(P<0.05);治疗后1个月联合组血清转铁蛋白水平低于参照组(P<0.05)。结论:体表定位丘脑血肿引流术+侧脑室穿刺引流术治疗丘脑出血破入脑室患者的临床效果显著,可缩短带管时间,改善脑组织损伤,促进患者恢复,减少感染发生。Objective:To study the clinical effect of thalamic hematoma drainage combined with bilateral ventricular puncture drainage in the treatment of Thalamic Hemorrhage Ruptured into ventricles.Methods:92 patients with thalamic hemorrhage rup-tured into ventricles in the hospital from April,2017 to April,2019 were divided into combined group(n=46)and control group(n=46)according to different surgical procedures.The control group was treated with bilateral ventricular puncture and drainage,and the combined group was treated with thalamic hematoma drainage based on the reference group.The time of ventricular drain-age tube,the incidence of intracranial infection and serum transferrin level of two groups were compared.Results:The time of ven-tricular drainage tube in the combined group was shorter than that in the control group(P<0.05).The incidence of intracranial in-fection in the combined group was 4.35%,which was lower than that in the control group 17.39%(P<0.05).Conclusion:The treatment of thalamic hematoma drainage combined with bilateral ventricular puncture and drainage at the top of skull can shorten the time of catheterization,promote the recovery of patients,and reduce the incidence of infection.
