摘　　要：目的:探讨心胸外科患者术后应激性高血糖的发生及对术后伤口愈合的影响。方法:回顾性分析2016年2月-2019年8月在广西医科大学第一附属医院心胸外科接受手术治疗的262例患者的临床资料。结果:262例患者中有73例发生术后应激性高血糖(静脉血糖≥11.1mmol/L),发生率为27.9%。其中,单因素分析结果表明,年龄、糖尿病史及术后应激性高血糖与术后伤口愈合时间相关,性别、手术方式对于术后伤口愈合时间影响不大;多因素分析结果表明,年龄(≥50岁)及术后高血糖水平均为影响术后伤口愈合的独立危险因素。结论:心胸外科患者术后应激性高血糖发病率超过1/4,对于年龄偏大(≥50岁)及发生术后血糖水平较高的患者应注意密切监控血糖水平,避免术后并发症的发生。Objective:To investigate the incidence of postoperative stress hyperglycemia in patients undergoing cardiothoracic surgery and its effect on postoperative wound healing.Methods:Retrospective analysis of the clinical data of 262 patients undergoing surgery in cardiothoracic surgery in the First Affiliated Hospital of Guangxi Medical University from February,2016 to August,2019 was done.Results:Postoperative stress hyperglycemia(venous blood glucose≥11.1 mmol/L)occurred in 73 of 262 patients,with an incidence of 27.9%.Univariate analysis showed that age,diabetes history and postoperative stress hyperglycemia were related with postoperative wound healing time.Gender and surgical methods had little effect on postoperative wound healing.Multivariate analysis showed that age(≥50)and postoperative hyperglycemia are independent risk factors for postoperative wound healing.Conclusion:The incidence of postoperative stress hyperglycemia in patients undergoing cardiothoracic surgery is more than 1/4.For patients with older age(≥50 years)and higher postoperative blood glucose levels,patients should pay close attention to blood glucose levels to avoid postoperative complications.
