中药熏洗联合针灸对痔疮患者肛门疼痛及TNF-α、CRP水平的影响

摘　　要：目的探究中药熏洗联合针灸对痔疮患者肛门疼痛及TNF-α、CRP水平的影响。方法回顾性分析2018年6月—2019年6月收治的120例痔疮患者的临床资料,根据治疗方式不同将其分为观察组(中药熏洗联合针灸治疗)与对照组(针灸治疗),各60例,均治疗14 d。比较2组患者治疗14 d后临床疗效,治疗前及治疗2、7、14 d后2组患者NRS疼痛评分,治疗前、治疗14 d后2组患者血清TNF-α、CRP水平与治疗后1、3、6个月2组患者复发率。结果治疗14 d后,观察组总有效率为98.33%高于对照组71.67%(P<0.05);与治疗前相比,治疗2、7、14 d后2组NRS疼痛评分均明显降低,且观察组患者治疗2、7、14 d后NRS疼痛评分均低于对照组(P<0.05);治疗后1、3、6个月,观察组复发率分别为0、1.69%、5.08%,均低于对照组2.33%、9.30%、23.26%,且治疗后6个月2组间差异有统计学意义(P<0.05)。结论中药熏洗联合针灸治疗痔疮患者疗效显著,可快速减轻患者肛门疼痛,降低血清TNF-α、CRP水平,减少炎症反应,降低复发率。Objective To explore the effect of traditional Chinese medicine fumigation and washing combined with acupuncture treatment on anal pain and TNF-α,CRP level in patients with hemorrhoids.Methods The clinical data of 120 cases of hemorrhoids treated from June 2018 to June 2019 were retrospectively analyzed.According to the different treatment methods,they were divided into observation group(Chinese medicine fumigation and washing combined with acupuncture treatment)and control group(acupuncture treatment),each of 60 cases were treated 14 d.Comparison of the clinical efficacy of the two groups after 14 d treatment,the pain score was NRS before treatment,2,7,14 d after treatment,and the serum TNF-αand CRP level of the two groups before treatment,14 d after treatment and the recurrence rate of the two groups after 1,3,6 months after treatment was carried out.Results The total effective rate of the observation group was 98.33%,which was higher than that of the control group(71.67%)(P<0.05).Compared with before treatment,the NRS pain score of the two groups after 2,7,14 d of treatment was significantly reduced,and the NRS pain score of the observation group after 2,7,14 d of treatment was lower than that of the control group(P<0.05).At 1,3 and 6 months after treatment,the recurrence rates of the observation group were 0.00%,1.69%and 5.08%,respectively,which was lower than those of the control group 2.31%,9.30%and 23.25%,and the differences between the two groups were statistically significant(P<0.05).Conclusion The curative effect of traditional Chinese medicine fumigation and acupuncture on hemorrhoids patients is remarkable,which can quickly relieve anal pain,reduce serum TNF-α,CRP level,reduce inflammatory reaction and reduce recurrence rate.
