上肢涡流气压水疗对上肢烧伤术后瘢痕增生的疗效

摘　　要：目的观察上肢涡流气压水疗对上肢烧伤术后瘢痕增生挛缩患者的疗效。方法将60例上肢烧伤术后瘢痕增生挛缩患者随机分为常规康复治疗组(A组,n=30)和上肢涡流气压水疗加常规康复治疗组(B组,n=30),予以相应的方法治疗2个疗程,治疗前后评估每组患者温哥华瘢痕量表、日常生活能力改良Barthel指数评分、上肢关节(腕关节、肘关节)活动度和VAS疼痛评分。结果2组患者治疗后的温哥华瘢痕量表、日常生活能力改良Barthel指数评分、上肢关节活动度和VAS疼痛评分,与治疗前相比均有不同程度的改善,差异有统计学意义(P<0.05或P<0.01);治疗后B组的温哥华瘢痕量表、日常生活能力改良Barthel指数评分、上肢关节活动度、VAS疼痛评分改善优于A组,具有统计学意义(P<0.05)。结论上肢涡流气压水疗加常规康复能改善上肢烧伤术后患者温哥华瘢痕量表、日常生活能力改良Barthel指数评分、上肢各关节活动度、VAS疼痛评分等各指标,具有临床意义。Objective To observe the effect of eddy current air pressure hydrotherapy on scar hyperplasia and contracture after surgery for upper limb burn injury.Methods Sixty patients with scar hyperplasia and contracture after upper limb burn surgery were randomly given conventional rehabilitation treatment alone(group A,n=30)or in combination with eddy current air pressure hydrotherapy(group B,n=30)for 2 courses.The Vancouver Scar Scale score,modified Barthel index score of activities of daily living,visual analog scale(VAS)score,and range of motion of the upper limb joints(wrist joint,elbowjoint)were measured before and after treatment.Results After treatment,the Vancouver Scar Scale score,modified Barthel index score of activities of daily living,range of motion of upper limb joints,VAS pain score and other parameters were improved in both groups(P<0.05 or P<0.01).Furthermore,all of these indicators were ameliorated in group B,compared with group A(P<0.05).Conclusion Eddy current air pressure hydrotherapy combined with routine rehabilitation can improve Vancouver Scar Scale score,modified Barthel index score of activities of daily living,range of motion of upper limb joints,VAS pain score and other indicators after surgery for upper limb burn injury.Therefore,the combined treatment is of clinical significance.
