彩超引导下多孔硅胶管引流联合手术治疗难治性肉芽肿性乳腺炎脓肿期价值分析

摘　　要：目的探讨彩超引导下多孔硅胶管引流联合手术治疗难治性肉芽肿性乳腺炎脓肿期的价值。方法本研究回顾性分析2014年7月至2018年2月在本院治疗的肉芽肿性乳腺炎脓肿期患者,从中选取356例患者,均行脓肿切开引流术。根据引流方式不同分为多孔硅胶管置管引流+病灶清除术组(硅胶管引流)178例,对照组选取采用传统纱条引流+病灶清除术组(纱条引流)178例。比较两组多次脓肿切开引流率、单次换药时间、病灶切除术术前换药时间、病灶切除术手术时间、术后乳房外形满意度评分、术后复发率的差异。结果彩超引导下多孔硅胶引流管置管术后脓肿再发率、换药时间、病灶切除术术前换药时间、术后乳房外形满意度评分、术后复发率的差异结果明显优于传统纱条引流组,差异具有统计学意义(P<0.05);两组行病灶切除术手术时间无明显差别,差异无统计学意义(P>0.05)。结论彩超引导下多孔硅胶管引流联合手术治疗肉芽肿性乳腺炎脓肿期患者可缩短病程,提高治疗效果。Objective:To investigate the value of porous silicone tube drainage combined with surgery in the treatment of the abscess period of granulomatous mastitis.Methods:A retrospective analysis was conducted.A total of 356 cases of granulomatous mastitis abscess in our hospital was included from July 2014 to February 2018.All of them were accepted abscess incision drainage.178 cases in treatment group accepted porous silicone tube catheter drainage before lesion resection(silicone tube drainage group),and the other 178 cases in control group was accepted traditional yarn drainage before lesion resection(yarn drainage group).The follow indexes were compared between the two groups:the incidence of re-incision of abscess,the mean time of dressing change,the interval between incision of abscess and lesion resection,the operative time of lesion resection,the satisfaction mean score of breast appearance after operation,and the incidence of recurrence.Results:The incidence of re-incision of abscess and recurrence were lower in the silicone tube drainage group than that in the yarn drainage group(P<0.05).The mean time of dressing change and the interval between incision of abscess and lesion resection were also less in the silicone tube drainage group(P<0.05).The satisfaction mean score of breast appearance after operation was higher in the silicone tube drainage group(P<0.05).There was no significant difference in the operative time of lesion resection between the two groups(P>0.05).Conclusion:The use of silicone tube drainage combined surgery could narrow the period of abscess of granulomatous mastitis and improve the therapeutic effectiveness,so it is worth popularizing.
