替加环素致低纤维蛋白原血症回顾性分析

摘　　要：目的:分析替加环素致低纤维蛋白原血症的病例及其临床特点,为临床安全用药提供参考。方法:通过检索PubMed数据库、万方数据库,收集替加环素致低纤维蛋白原血症的不良反应个案报道,并对患者性别、年龄、合并基础疾病、联合用药,以及低维蛋白原血症发生时间、临床表现及预后等进行统计分析。结果:共检索到11例独立文献报道,患者多为男性,偏老龄。11例病例中5例采用常规剂量治疗,3例采用较高剂量治疗。11例患者中有8例使用替加环素后纤维蛋白原FIB低于1 g·L-1,3例FIB介于1~2 g·L-1。发生低纤维蛋白原血症的时间多介于3~10 d,多数患者在停药、对症治疗3~7 d后指标基本恢复正常。结论:替加环素致低纤维蛋白原血症的不良反应较少,但后果严重,应引起临床重视。OBJECTIVE To analyze the general patterns and characteristics of hypofibrinogenemia related to tigecycline and provide a reference for safety of clinical medication.METHODS By searching PubMed and Wanfang databases, the adverse reactions of tigecycline induced hypofibrinogenemia were collected, and patients’ sex, age, basic diseases and combined medication, the occurrence time, clinical manifestation and prognosis were analyzed statistically.RESULTS A total of 11 independent literature reports were retrieved, and the most of the patients were male and elderly. 5 of the 11 cases were treated with conventional tigecycline and 3 with higher dose. Among the 11 patients, FIB was lower than 1 g·L-1 in 8 cases and 1-2 g·L-1 in 3 cases. The time of occurrence of hypofibrinogenemia was usually 3-10 days, and most patients returned to normal after 3~7 days of drug withdrawal and symptomatic treatment.CONCLUSION The side effects of tigecycline on hypofibrinogenemia were few, but the consequences are serious and should be paid attention to in clinical practice.
