整建制下新型冠状病毒肺炎危重症患者的护理管理

摘　　要：目的总结整建制下定点医院收治新型冠状病毒肺炎危重症患者的护理管理措施。方法 11支援鄂医疗队整建制接管11个隔离病区,会同医院3支护理队伍,通过实行强化培训、"一医一护"管理模式,构建整建制接管新模式,规范病区管理方式,成立定点医院联合护理部,实行护理质量安全管理,为危重症患者提供优质护理服务,全面开展人文关怀工作,关注患者心理健康。结果 2020年2月13日至3月15日,1 531名护士零感染。累计收治患者1 022人,其中:危重症489人,占比47.8%,危重症转轻症占比34.1%,治愈出院763名。心理疏导小分队共完成832名患者的心理评估。收到患者感谢信400余封,住院患者满意度平均分高达99.36分。完成出院新冠肺炎患者电话回访229人,护理工作获得患者广泛好评,高度赞赏。结论整建制接管有利于高效救治新冠肺炎患者,对降低病亡率也起到非常重要的作用。同时成立联合护理部,规范护理行为,制定并实施各项护理管理制度,保证各隔离病区的正常运转,为新冠肺炎患者提供优质护理服务,获得患者广泛好评。Objective To summarize nursing management measures for critically ill patients with COVID-19 in a designated hospital under the integrated system.Methods The 11 medical assistance squads to Hubei took over 11 isolated wards together with the 3 nursing teams of the hospital.Through the implementation of intensive training,the management mode of"one medical care and one nursing care",a new mode of integrated system,was established by standardizing ward management,setting up joint nursing department of designated hospitals and implementing the management of nursing quality and safety,in order to provide high-quality care services for critically ill patients,carry out humanistic care,and focus on patients’mental health.Results None of the 1,531 nurses were infected from February 13 to March 15,2020.A total of 1,022 patients were admitted and treated,of which 489 were critically ill,accounting for 47.8%,34.1%transferred from critical to mild,and 763 cured and discharged.The psychopathology group completed psychological assessments of 832 patients.More than 400 thank-you letters were received from patients,and the average score of inpatient satisfaction was 99.36.The telephone follow-up of 229 COVID-19 patients were completed after discharge,and the nursing work was widely praised and highly appreciated by the patients.Conclusion The whole system is conducive to the efficient treatment of COVID-19 patients,and plays a crucial role in reducing mortality.In addition,a joint nursing department was established to standardize nursing behaviors,formulate and implement various nursing management systems,thus ensuring the normal operation of each isolated ward,and providing high-quality nursing services for COVID-19 patients,which were widely acclaimed by patients.
