摘　　要：目的:探讨FOCUS-PDCA管理工具在降低药房调配差错率中的效果。方法:抽取阿里地区人民医院2018-2019年共105 501张处方,以该院药房调配差错率高问题为导向,成立CQI小组依照FOCUS-PDCA中寻找、组织、明确、理解、选择、计划、实施、检查、执行这9个环节,采用图表、流程图、鱼骨图、柏拉图、甘特图、雷达图等工具从找出问题、分析原因、解决问题三方面完成对核心问题具有深度地和广度的彻底解决;同时,以对比2018年与2019年调配差错率来评价FOCUS-PDCA在降低药房调配差错率中的成效,优化调配制度流程从而实现降低处方调配差错率。结果:(1)造成我院药房调配差错率高的原因存在4个方面:(1)制度层面;(2)人员层面;(3)信息系统层面;(4)环境层面。(2)活动后成果显著:(1)阿里地区人民医院药房调配差错率由10.2/10 000降低至2.6/10 000(P<0.001);(2)组员圈能力方面也有大幅度的提高。结论:FOCUS-PDCA在降低阿里地区人民医院药房调配差错率中成效显著。OBJECTIVE To explore the effect of FOCUS-PDCA management tool in reducing the error rate of pharmacy allocation.METHODS A total of 105,501 prescriptions from 2018 to 2019 in Ali District People’s Hospital were selected.Guided by the high error rate of pharmacy dispensing in the hospital,a CQI group was established to identify,organize,clarify,understand,select,plan,implement,inspect and implement these nine links in accordance with FOCUS-PDCA.Chart,flow chart,fishbone diagram,plato,Gantt diagram,radar diagram and other tools were used to complete the thorough solution of the core problems in three aspects:identifying the problems,analyzing the causes and solving the problems;at the same time,the effectiveness of FOCUS-PDCA in reducing the error rate of pharmacy dispensing was clarified by comparing the error rate in 2018 and 2019,and the dispensing system process was optimized to achieve the reduction of error rate of prescription dispensing.RESULTS 1.There are four reasons for the high error rate in dispensary of our hospital:(1)system level(2)personnel level(3)information system level(4)environment level.CONCLUSION FOCUS-PDCA is effective in reducing the error rate of pharmacy allocation in Ali District People’s Hospital.
