不同术式对小儿腹股沟疝修补术的临床价值及安全性对比研究

摘　　要：目的:对比传统开放式疝囊高位结扎术和经脐单孔腹腔镜下疝囊高位结扎术在小儿腹股沟疝治疗中的临床价值及安全性.方法:选取2016年9月-2018年9月于本院就诊的腹股沟疝患儿90例作为研究对象,按手术方法不同将其分为观察组(n=47)与对照组(n=43).对照组采用传统开放式疝囊高位结扎术治疗,观察组采用经脐单孔腹腔镜下疝囊高位结扎术治疗.比较两组临床疗效、临床相关指标、术后疼痛程度、并发症发生情况及复发率.结果:两组治疗总有效率比较,差异无统计学意义(P>0.05).观察组手术时间、术中出血量、住院天数及切口长度均低于对照组(P<0.05).观察组术后1 d、1个月及6个月视觉模拟评分(visual analogue score,VAS)均低于对照组(P<0.05).两组并发症发生率和复发率比较,差异均无统计学意义(P>0.05).结论:传统开放式疝囊高位结扎术和经脐单孔腹腔镜下疝囊高位结扎术治疗小儿腹股沟疝的效果均满意,但经脐单孔腹腔镜下疝囊高位结扎术可缩短手术时间与住院时间,减少切口长度,且减轻术后疼痛,值得在临床中推广及应用.Objective:To compare the clinical value and safety of traditional open hernia sac high ligation and transumbilical single-hole laparoscopic hernia sac high ligation in the treatment of inguinal hernia in children.Method:A total of 90 children with inguinal hernia admitted to our hospital from September 2016 to September 2018 were selected as study subjects.They were divided into observation group(n=47)and control group(n=43)according to different surgical methods.The control group was treated with the traditional open hernia sac high ligation,while the observation group was treated with transumbilical single-hole laparoscopic hernia sac high ligation.Clinical efficacy,clinical indicators,postoperative pain degree,complications and recurrence rates were compared between two groups.Result:There was no statistically significant difference in total effective rate between two groups(P>0.05).The operative time,intraoperative blood loss,length of hospital stay and incision length in the observation group were lower than those in the control group(P<0.05).Visual analogue score(VAS)in the observation group were lower than those in the control group at 1 d,1 month and 6 months after surgery(P<0.05).There were no statistically significant differences in the incidence of complications and recurrence between the two groups(P>0.05).Conclusion:Both the traditional open hernia sac high ligation and the transumbilical single-hole laparoscopic hernia sac high ligation have satisfactory results in the treatment of inguinal hernia in children.However,the traditional open hernia sac high ligation can shorten the operative time and hospital stay,reduce the incision length,and alleviate postoperative pain,which is worthy of promotion and application in clinical practice.
