摘　　要：目的:探讨N末端B型脑钠肽前体(NT-proBNP)在缩窄性心包炎术后疗效评价中的应用价值.方法:选取2016年6月-2019年6月本院收治的缩窄性心包炎患者60例,所有患者入院后均行心包剥脱术进行治疗.根据患者术后1年内主要心血管不良事件(MACE)发生情况将其分为无MACE组34例与MACE组26例.记录并比较两组患者手术前后不同时间点血清NT-proBNP水平、左室射血分数(LVEF)、肺动脉平均压、中心静脉压数值.结果:术后1 d、1周、3个月、1年,两组血清NT-proBNP水平、肺动脉平均压、中心静脉压数值均明显依次降低(P<0.05).术后1 d、1周、3个月、1年,两组LVEF均明显依次升高(P<0.05).两组的组间、不同时点以及组间和不同时点的交互作用均差异明显(P<0.05).Spearman相关性分析表明缩窄性心包炎行心包剥脱术患者血清NT-proBNP水平与LVEF呈负相关性(P<0.05)、与肺动脉平均压及中心静脉压数值分别呈正相关性(P<0.05).结论:缩窄性心包炎患者血清NT-proBNP水平明显上升,术后逐渐下降至正常水平,可作为对缩窄性心包炎术后疗效评价的参考指标.Objective:To discuss the application value in the evaluation of postoperative efficacy of N-terminal pro brain natriuretic peptide precursor(NT-proBNP)in patients with constrictive pericarditis.Method:A total of 60 patients with constrictive pericarditis admitted to our hospital from June 2016 to June 2019 were selected.After admission,all patients underwent pericardiectomy for treatment.According to the occurrence of major cardiovascular adverse events(MACE)within 1 year after surgery,the patients were divided into 34 cases in MACE free group and 26 cases in MACE group.Serum NT-proBNP levels,left ventricular ejection fraction(LVEF),pulmonary artery mean pressure,and central venous pressure were recorded and compared between two groups at different time points before and after surgery.Result:At 1 d,1 week,3 months and 1 year after surgery,serum NT-proBNP level,mean pulmonary artery pressure and central venous pressure in two groups were significantly decreased successively(P<0.05).At 1 d,1 week,3 months and 1 year after surgery,LVEF in two groups were significantly increased successively(P<0.05).The interaction between two groups at different time points,as well as between and at different time points were significantly different(P<0.05).Spearman correlation analysis showed that serum NT-proBNP level in patients with constrictive pericarditis underwent pericardial extubation was negatively correlated with LVEF(P<0.05),and positively correlated with pulmonary artery mean pressure and central venous pressure(P<0.05).Conclusion:The serum NT-proBNP level in patients with constrictive pericarditis increase significantly,and gradually decrease to normal level after operation,it can be used as a reference index for evaluating the curative effect of constrictive pericarditis.
