早期采用小剂量肝素治疗新生儿硬肿症的出凝血功能变化及其临床意义

摘　　要：探讨早期采用小剂量肝素治疗新生儿硬肿症不同阶段的出凝血功能变化及其临床意义。选取新生儿科收治的52例新生儿硬肿症作为观察组,选取52例同期足月生产且体重正常的新生儿作为对照组,比较两组新生儿凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FBI)及血小板计数(PLT)等出凝血功能指标的阳性率,观察组不同分级的硬肿症患儿小剂量肝素治疗效果及发生弥散性血管内凝血(DIC)、肺出血及死亡等不良预后的发生率。观察组的出凝血功能指标PT、APTT、FBI及PLT阳性率均显著高于对照组(P<0.05)。新生儿硬肿症分级增高,患儿出凝血功能指标PT、APTT、FBI及PLT阳性率也明显升高(P<0.05),小剂量肝素治疗的总有效率显著降低(P<0.05),患DIC、肺出血等并发症的发生率及死亡率显著升高(P<0.05)。新生儿硬肿症分级越高,出凝血功能异常越严重,DIC及肺出血等并发症的发生率及死亡率也越高。早期采用小剂量肝素治疗可显著改善硬肿症患儿的出凝血功能,降低DIC及肺出血等并发症的发生率及死亡率,且越早治疗效果越好。The research explores the coagulation function change and clinical significance of small dose heparin in early stage treatment on newborns with scleredema.52 newborns with scleredema in Neonatology Department are regarded as observation group,and 52 newborns of full term production and normal weight as observation group.The positive rates of coagulation function index,such as PT,APTT,FBI and PLT of two groups are compared.The treatment effect of small dose heparin treatment on newborns with scleredema,and poor prognosis occurrence rates of DIC and pneumorrhagia of different degrees are observed.Coagulation function indexes,including the positive rates of PT,APTT,PBI and PLT,of observation group are significantly higher than control group(P<0.05).The degree of scleredema of newborns increases,and the positive rates of coagulation function indexes,such as PT,APTT,FBI and PLT,increase significantly(P<0.05).Total effective rate of small dose heparin treatment decreases significantly(P<0.05).The occurrence rate and mortality rate of DIC and pneunorrhagia increase significantly(P<0.05).The higher the scleredema grading is,more serious dysfunction of coagulation becomes,and the higher the DIC and pneumorrhagia occurrence rates and mortality are.Small dose heparin in early stage treatment on newborns with scleredema can significantly improve the coagulation function of newborns with scleredema,and reduce the occurrence rate and mortality of complications of DIC and pneumorrhagia.The early the treatment is,the better the effect is.
