摘 要:目的甲状旁腺功能损伤及术后淋巴结复发转移是甲状腺癌术后的关键问题。本临床回顾性分析纳米碳对于高危分化型甲状腺肿瘤患者行全甲切除及中央区淋巴结清扫(central lymph node dissection以下称CLND)的应用价值。方法收集我科2017年10月31日—2019年10月31日术后病理为高危分化型的甲状腺癌病例86例。按是否术中注射纳米碳分为两组。分析术前、术后1/2 h、术后45 d Ca^2+及PTH,CLND数目,CLN转移数目,腺体癌灶数目,手术时长,手术出血量、标本见甲状旁腺组织病例例数,口角手足麻木抽搐例数情况。计算两组间相关数据。分析在全甲切及CLND中纳米碳使用对治疗效果的影响。结果注射纳米碳组与未注射组在术后1/2 h Ca^2+、术后1/2 h PTH、CLND数目、旁腺误切例数、口角手足麻木抽搐例数差异具有统计学意义,P<0.05。在性别、岁数、术前Ca2+及PTH、术后45 d Ca2+及PTH、手术时长、出血量水平观察中差异无统计学意义,P>0.05。结论术中进行纳米碳混悬液的应用在高危分化型甲状腺癌行全甲切除及CLND中更有利于旁腺保护及CLND。Objective Thyroidectomy is the main method for the treatment of thyroid tumor.This clinical retrospective analysis nano-carbon was conducted on the application value of total nail resection and central lymph node dissection(CLND)in patients with high-risk differentiated thyroid tumors.Methods From October 31,2017 to October 31,2019,86 cases of thyroid cancer with high-risk differentiation were collected.The patients were divided into two groups according to whether nano carbon was injected during operation.The Ca2+and PTH,the number of CLND,the number of CLN metastasis,the number of gland cancer,the operation time,the amount of bleeding,the number of parathyroid tissue cases,the number of numbness and convulsion of mouth,hand and foot were analyzed before operation,1/2 h and 45 d after operation.The related data between the two groups were calculated.Objective to analyze the effect of nano carbon in total thyroidectomy and CLND.Results There were statistically significant differences in the number of 1/2 h Ca2+,1/2 h PTH,CLND,parathyroidectomy,and numbness and convulsions of mouth,hand and foot between the injected group and the non-injected group,P<0.05.Among them,there were no statistical differences in gender,age,preoperative Ca2+and PTH,postoperative 45 d Ca2+and PTH,operative duration and blood loss level,P>0.05.Conclusion The application of nano-carbon suspension during the operation is more beneficial to the parathyroid protection and CLND in the treatment of high-risk differentiated thyroid carcinoma with total thyroidectomy and CLND.
