探讨免疫组化在炎性肌纤维母细胞肿瘤病理诊断中的价值

摘　　要：目的探究炎性肌纤维母细胞肿瘤(inflammatory myofibroblastic tumor,IMT)采用免疫组化进行病理诊断的使用价值。方法本研究于近2年时间(2017年8月—2019年8月)时间段接诊的IMT患者共60例予以研究,分别对这60例IMT患者的免疫组化抗体P53、ALK、组织学形态、临床资料以及cyokeratin(AE/AE3)等实施染色分析。结果60例患者其肿瘤组织的具体表现以灰白色为主,而其形状则分为以下两种,分别为结节状以及分叶状。其中的分叶状所占比例为58.33%(35/60),结节状所占比例为41.67%(25/60);肿瘤的质地坚实,其直径是3~7 cm,其中有3例(5.00%)患者经诊断显示肿瘤肿块以出血或者是钙化表现出;以上收治的60例患者全部以波形蛋白弥漫强阳性表达,其中有54例(90.00%)患者以平滑肌肌动蛋白表达,与此同时,这80例患者Ki67指数参数偏高,和正常标准增值幅度对比存在20%左右,并且间变性大细胞淋巴瘤基酶胞质呈现阳性。结论组织学作为IMT病理诊断的重要基础,联合免疫组化抗体综合采用,特别是针对P53、AE/AE3和ALK进行诊断分析具有不可取代的价值地位。Objective To explore the application value of immunohistochemistry in pathological diagnosis of inflammatory myofibroblastic tumor(IMT).Methods A total of 60 IMT patients admitted in the past 2 years(from August 2017 to August 2019)were studied in this study.The immunohistochemical antibodies P53,ALK,histological morphology,clinical data and cyokeratin(AE/AE3)of these 60 IMT patients were analyzed by staining.Results The tumor tissues of 60 patients were mainly gray-white,and their shapes were divided into the following two types,nodular and lobulated respectively.Among them,lobulation accounted for 58.33%(35/60)and nodule 41.67%(25/60).The texture of the tumor is firm,with the minimum diameter of 3~7 cm.Among them,3 patients(5.00%)showed hemorrhage or calcification as the tumor mass after diagnosis.All of the 60 patients treated above were strongly and diffusely expressed vimentin,of which 54 patients(90.00%)expressed smooth muscle actin.At the same time,the Ki67 index parameter of these 80 patients was significantly higher,about 20%higher than the normal standard increment,and the cytoplasm of anaplastic large cell lymphoma-based enzyme was positive.Conclusion Histology,as an important basis for pathological diagnosis of IMT,combined with immunohistochemical antibody,especially for P53,AE/AE3 and ALK,has irreplaceable value.
