摘　　要：目的探讨经阴道耻骨后无张力尿道中段悬吊术(tension-free vaginal sling after pubic bone,TVT)治疗女性压力性尿失禁(stress urinary incontinence,SUI)的临床效果及并发症发生情况。方法选取2016年1月—2019年3月我院收治的19例女性SUI患者,均行TVT治疗,且由同一个手术者完成;其中6例阴道壁膨出患者同时行阴道壁修补术,观察其手术时间、术中出血量、手术效果及并发症等情况。结果19例均顺利完成手术,手术时间27~107 min,中位数为45 min,术中出血量2~100 mL,中位数为20 mL,均未出现脏器损伤、血肿及感染;2例出现尿潴留,1例予重置尿管、1例予药物对症处理后均治愈;19例均顺利出院,并于术后1个月、3个月及12个月通过门诊复查及电话随访,均未出现并发症及复发。结论TVT操作简便、并发症少、恢复快,且效果显著。Objective To investigate the clinical effect and complications of transvaginal pubic tension-free urethral suspension(TVT)for female stress urinary incontinence(SUI).Methods From January 2016 to March 2019,19 female patients in our hospital were treated with TVT and completed by the same surgeon;Among them,6 cases of vaginal wall prolapse were treated with vaginal wall repair at the same time.The operation time,intraoperative blood loss,operation effect and complications were observed.Results All 19 patients successfully completed the operation.The operation time was 27~107 minutes,with the median of 45 minutes.The intraoperative blood loss was 2~100 mL,and the median was 20 mL..No organ damage,hematoma,or infection occurred;There were 2 cases of urinary retention,1 case was placed in the urinary catheter again,and 1 case was treated with symptomatic drugs,and all were cured 19 cases were successfully discharged from the hospital,and were reviewed by clinic and telephone follow-up at 1 month,3 months,and 12 months after surgery.No complications or recurrence occurred.Conclusion TVT is easy to operate,has few complications,quick recovery,and has significant effects.
