摘　　要：目的:观察杵针大椎八阵穴、河车脑椎段、河车椎至段及河车阳命段治疗颈型颈椎病的临床疗效。方法:收集2018年3月—2019年6月上海市长宁区虹桥街道社区卫生服务中心及3个卫生服务站门诊收治的颈型颈椎病(风寒湿型)患者100例,随机分为治疗组及对照组各50例。治疗组采用金刚杵,取大椎八阵穴、河车脑椎段、河车椎至段及河车阳命段,应用点叩法、升降法及开阖法治疗5 min,1次/d,5 d为1个疗程,治疗2个疗程。对照组取大椎穴、风池穴、颈夹脊穴进行传统针刺20 min,1次/d,5 d为1个疗程,治疗2个疗程。通过McGill疼痛问卷表评估改善情况,观察临床疗效。结果:临床疗效评价结果显示观察组治愈率为18%,显效率为52%,有效率为15%;对照组治愈率为4%,显效率为19%,有效率为27%。治疗组的临床愈显率为70%,优于对照组的46%(P<0.05)。治疗后,两组McGill疼痛评分均较同组治疗前有所改善(P<0.05),但组间差异无统计学意义(P>0.05)。结论:太极杵针技术治疗颈型颈椎病具有疗效显著,操作简便,安全无副作用,依从性强等特点。Objective:To observe the clinical curative effect of pestle acupuncture at Dazhuibazhen acupoint,Hechenao vertebral segment,Hechechuizhi segment and Hecheyangming segment in the treatment of cervical spondylosis of cervical type.Methods:From March 2018 to June 2019,100 patients with cervical spondylosis of cervical type(wind-cold-dampness type)were collected from the community health service center and 3 health service stations in Hongqiao Comunity of Changning District,Shanghai,and were randomly divided into a treatment group and a control group with 50 each.In the treatment group,the vajra pestle in the Taiji pestle needle was used,and Dazhuibazhen acupoint,Hechenao vertebral segment,Hechechuizhi segment and Hecheyangming segment were selected with application of tapping method,flexion-extension movement and opening and closing method for 5 minutes,1 time/d,5 days as a course of treatment,and the treatment was 2 courses.The control group were treated with traditional acupuncture of Dazhui,Fengchi,and Cervical Jiaji points for 20 min,1 time/d,5 days as a course of treatment,and the treatment was 2 courses.The improvement was evaluated by McGill pain questionnaire and the clinical efficacy was observed.Results:The clinical efficacy evaluation results showed that the observation group had the cure rate of 18%,the markedly effective rate of 52%,and the effective rate of 15%;in the control group,the cure rate was 4%,the markedly effective rate was 19%,and the effective rate was 27%.The clinical effective rate of the treatment group was 70%,which was better than 46%of the control group(P<0.05).After treatment,the McGill pain scores of the two groups were improved compared with the same group before treatment(P<0.05),but the difference between the groups was not statistically significant(P>0.05).Conclusion:The Taiji pestle needle technique in the treatment of cervical spondylosis of cervical type has the characteristics of remarkable curative effect,simple operation,safety without side effects,and strong compliance.
