抗C1q抗体在狼疮肾炎血清中的表达及临床价值

摘　　要：目的探讨系统性红斑狼疮肾炎患者血清中抗C1q抗体的表达及检测结果在临床中的价值。方法选取绵阳市中心医院2018年11月至2019年6月收治的68例狼疮患者作为研究对象并设为观察组,选择同期健康体检者68例设为健康对照组,再选择同期自身免疫疾病者68例设为疾病对照组,采用酶联免疫吸附测定法(ELISA法)检测血清抗C1q抗体,比较三组患者血清中抗C1q抗体阳性率及不同系统性红斑狼疮肾炎病理分型患者中抗C1q抗体阳性率的情况。结果观察组血清中抗C1q抗体的阳性率为51.47%,高于疾病对照组7.35%和健康对照组0.00%(P<0.05);系统性红斑狼疮肾炎IV型的抗C1q抗体阳性率相对高于其他病理分型(P<0.05)。结论血清抗C1q抗体的检测结果在一定程度上可以反映系统性红斑狼疮疾病的活动性,有效指导临床区分其病理分型,特别是对于Ⅳ型而言。Objective To investigate the expression of anti-C1q antibody in serum of patients with systemic lupus erythematosus nephritis and its clinical value.Methods A total of 68 lupus patients admitted to Mianyang Central Hospital from November 2018 to June 2019 were selected as the observation group,and 68 healthy persons and 68 patients with autoimmune diseases were selected as healthy control group and disease control group respectively.Serum anti-C1q was detected by ELISA.The positive rate of anti-C1q antibody in serum of three groups and the positive rate of anti-C1q antibody in patients with different pathological types of systemic lupus erythematosus nephritis were compared.Results The positive rate of anti-C1q antibody in the observation group was 51.47%,higher than that in the disease control group(7.35%)and the healthy control group(0.00%)(P<0.05);the positive rate of anti-C1q antibody in type IV of systemic lupus erythematosus nephritis was relatively higher than that of other pathological types(P<0.05).Conclusion The detection results of serum anti-C1q antibody can reflect the activity of lupus disease to a certain extent,and effectively guide the clinical differentiation of pathological classification,especially for type Ⅳ.
