NGF、TRPV1在子宫内膜异位症患者中的表达及与疼痛的关系

摘　　要：目的研究神经生长因子(nerve growth factor, NGF)及香草酸瞬时受体亚型1(transient receptor potential vanilloid 1, TRPV1)在子宫内膜异位症(endometriosis, EMs)患者在位内膜、异位内膜的表达及与疼痛的关系。方法选择2017年1月—2019年12月诊治的EMs患者50例作为观察组,根据视觉模拟评分法(visual analogue scale, VAS)对患者疼痛程度进行评分,其中无痛组14例,疼痛组36例,选择同期就诊的妇科良性病变且无痛经患者20例为对照组。检测观察组在位内膜、异位内膜及对照组内膜中NGF、TRPV1蛋白及mRNA的表达,分析EMs在位内膜和异位内膜NGF、TRPV1表达水平、VAS评分的相关性。结果观察组在位内膜NGF、TRPV1蛋白及mRNA表达高于对照组(P<0.01);疼痛组在位内膜NGF、TRPV1蛋白及mRNA表达水平高于无痛组及对照组(P<0.05,P<0.01),无痛组与对照组相比差异无统计学意义(P>0.05);疼痛组异位内膜NGF、TRPV1的表达水平高于无痛组(P<0.05)。EMs在位内膜和异位内膜中NGF与TRPV1表达水平呈正相关关系,二者与VAS评分亦呈正相关关系(P<0.05,P<0.01)。结论 NGF、TRPV1的过度表达与EMs疼痛的发生密切相关,NGF、TRPV1共同参与EMs疼痛的发生,且二者起到协同作用。Objective To investigate the expression of nerve growth factor(NGF)and transient receptor potential vanilloid 1(TRPV1)in eutopic and ectopic endometrium in endometriosis(EMs)patients and its relationship with EMs-associated pain.Methods Fifty EMs patients from January 2017 to December 2019 were selected as observation group.According to visual analogue scale(VAS),the pain degree of the patients was scored.Therefore,there were 14 cases in the painless group and 36 cases in the pain group.Another 20 cases with gynecological benign lesions and no dysmenorrhea who presented to the hospital during the same period were selected as control group.Protein and mRNA expressions of NGF and TRPV1 in eutopic endometrium and ectopic endometrium of observation group and in normal endometrium in the control group were detected.The correlation of the expression levels of NGF and TRPV1 with the VAS score in the eutopic and ectopic endometrium of EMs was analyzed.Results The protein and mRNA expressions of NGF and TRPV1 in eutopic endometrium were significantly higher in observation group than in control group(P<0.01).The protein and mRNA expressions of NGF and TRPV1 in eutopic endometrium in pain group were significantly higher than those in control and non-pain groups(P<0.05,P<0.01).No significant difference was found in non-pain group and control group(P>0.05).The protein and mRNA expressions of NGF and TRPV1 in ectopic endometrium were higher in pain group compared to non-pain group(P<0.05).There was a positive correlation between NGF and TRPV1 in the eutopic and ectopic endometrium of EMs.In addition,there were positive correlations of NGF and TRPV1 with VAS(P<0.05,P<0.01).Conclusion The overexpression of NGF and TRPV1 was closely related to the occurrence of EMs-associated pain.NGF and TRPV1 were involved in the occurrence of EMs-associated pain,playing a synergistic effect.
