穴位埋线法治疗癫痫临床研究的Meta分析

摘　　要：【目的】系统评价以穴位埋线为主的中医疗法治疗癫痫的临床疗效及安全性。【方法】计算机检索中国知网期刊全文数据库(CNKI)、维普数据库(VIP)、万方数据知识服务平台(Wanfang)及美国医学在线(PubMed)、ScienceDirect、国际循证医学图书馆(Cochrane Library)等中英文数据库。检索并纳入穴位埋线治疗癫痫的随机对照试验(RCT)。由2名研究者按照制定的纳入和排除标准独立筛选文献,提取资料,并利用Cochrane手册对纳入文献进行质量评估,采用Revman 5.3软件进行Meta分析。【结果】共纳入13项RCT研究,合计1378例患者。Meta分析结果显示:与西药组比较,穴位埋线组或穴位埋线加西药组能提高总有效率(OR=3.03,95%CI[1.94,4.73],P<0.00001),降低发作频率(WMD=-1.04,95%CI[-1.15,-0.92],P<0.00001),改善临床症状(WMD=-1.57,95%CI[-2.76,-0.38],P<0.00001)。【结论】穴位埋线法治疗癫痫临床疗效优于常规西药治疗,可改善临床症状和降低发作频率。但因纳入试验存在一定发表偏倚且质量偏低,故仍需多中心、大样本、高质量的RCT予以进一步验证。Objective To systematically evaluate the curative efficacy and safety of Chinese therapies dominated by acupoint catgut embedding in treating epilepsy.Methods Chinese and English databases such as CNKI,VIP,Wanfang,PubMed,ScienceDirect and Cochrane Library were searched out by computer for including randomized controlled trials(RCTs)on acupoint catgut embedding therapy for epilepsy.The literatures were screened out by two researchers independently according to the established inclusion and exclusion criteria for extracting the data,and then the included literatures were given quality evaluation by Cochrane manual,and Meta-analysis by Revman 5.3 software.Results A total of 13 RCTs,involving 1378 patients,were enrolled in studies.Metaanalysis results showed that compared with the western medicine group,the acupoint catgut embedding group and acupoint embedding plus western medicine group enhanced overall effective rate(OR=3.03,95%CI[1.94,4.73],P<0.00001),reduced the frequency of attack(WMD=-1.04,95%CI[-1.15,-0.92],P<0.00001),and improved the clinical symptoms(WMD=-1.57,95%CI[-2.76,-0.38],P<0.00001).Conclusion Acupoint catgut embedding therapy is superior to conventional western medicine in treating epilepsy,and is effective for improving clinical symptoms and reducing the frequency of attack.But due to certain publication bias and low quality in included test,multiple centers,big sample,and high quality of RCTs need to be verified.
