摘 要:目的探讨维生素D受体(VDR)基因多态性与儿童慢性乙型肝炎(CHB)易感性、乙型肝炎病毒(HBV)基因型、肝脏病变严重程度及血清25羟维生素D[25(OH)D]水平的关系。方法选择该院189例CHB患儿为CHB组和56例健康儿童为对照组,CHB患儿根据肝脏病变严重程度分为轻度组(78例)和中度组(42例),按HBV基因型分为B型组(118例)和C型组(21例)。检测所有儿童的VDR基因BsmI(rs1544410)、ApaI(rs7975232)、TaqI(rs731236)、FokI(rs2228570)位点多态性,并分析其与儿童CHB易感性、HBV基因型、肝脏病变严重程度及血清25(OH)D水平的关系。结果VDR基因ApaI位点的基因型在CHB组和对照组中的频率分布比较,差异有统计学意义(P<0.05),CHB组AC基因型(43.9%)频率分布高于对照组(26.8%),而BsmI、TaqI和FokI 3个位点的基因型和等位基因频率分布在两组中的比较,差异均无统计学意义(P>0.05);VDR基因BsmI、ApaI、TaqI、FokI 4个位点的基因型与等位基因频率分布在CHB患儿轻度组和中度组、HBV B型组和C型组中的比较,差异均无统计学意义(P>0.05);血清25(OH)D水平在VDR基因4个位点不同基因型间比较,差异均无统计学意义(P>0.05)。结论VDR基因ApaI位点多态性可能与儿童CHB易感性相关,未发现VDR基因位点BsmI、ApaI、TaqI、FokI的多态性与CHB患儿肝脏病变严重程度、HBV基因型及血清25(OH)D水平的相关性。Objective To investigate the relationship between vitamin D receptor(VDR)gene polymorphism and susceptibility to chronic hepatitis B(CHB),hepatitis B virus(HBV)genotype,severity of liver disease and serum 25 hydroxyvitamin D[25(OH)D]level in children.Methods A total of 189 cases of CHB children in our hospital were selected as CHB group and 56 healthy children as control group.CHB children were divided into mild group(78 cases)and moderate group(42 cases)according to the severity of liver lesions,and were divided into B type group(118 cases)and C type group(21 cases)according to HBV genotype.The VDR gene BsmI(rs1544410),ApaI(rs7975232),TaqI(rs731236),FokI(rs2228570)polymorphisms in all children were detected,and their relationship with CHB susceptibility,HBV genotype,severity of liver disease and serum 25(OH)D level were analyzed.Results The frequency distribution of ApaI genotype of VDR gene between CHB group and control group was statistically significant(P<0.05).The frequency distribution of AC genotype in CHB group(43.9%)was higher than that in control group(26.8%).There was no significant difference in genotype and allele frequency distribution of BsmI,TaqI and FokI between the two groups(P>0.05);there were no significant differences in genotype and allele frequency distribution of VDR gene BsmI,ApaI,TaqI,FokI in mild and moderate groups,HBV B group and C group(P>0.05);serum 25(OH)D level in different genotypes of VDR gene had no significant difference(P>0.05).Conclusion VDR gene ApaI polymorphism may be associated with the susceptibility to CHB in children.There is no correlation between VDR gene polymorphism BsmI,ApaI,TaqI,FokI and the severity of liver disease,HBV genotype and serum 25(OH)D level.
