摘  要:目的探讨转录因子7类似物2(TCF7L2)基因rs7903146位点基因多态性与2型糖尿病(T2DM)合并冠状动脉粥样硬化性心脏病(CHD)发病风险的相关性,寻找T2DM合并CHD的易感基因及影响因素。方法选取该院100例T2DM患者为T2DM组,100例CHD患者为CHD组,100例T2DM合并CHD患者为T2DM+CHD组,100例体检健康者为对照组,检测4组TCF7L2基因rs7903146位点基因多态性,分析基因型分布特点;比较4组不同基因型血脂、血糖等指标的差异;Logistic回归分析T2DM合并CHD的影响因素。结果TCF7L2基因rs7903146位点CC/CT基因型分布在各组间比较,差异均无统计学意义(P>0.05)。各组CC/CT基因型间血糖、血脂水平比较,差异均无统计学意义(P>0.05)。Logistic回归分析结果显示,高龄、高水平糖化血红蛋白是T2DM合并CHD的独立危险因素,低水平高密度脂蛋白胆固醇是预防T2DM合并CHD的保护因素,而rs7903146位点基因型不是发生T2DM合并CHD的影响因素。结论TCF7L2基因rs7903146位点基因多态性与T2DM合并CHD发病风险,血糖、血脂水平无关。Objective To investigate the relationship between the rs7903146 polymorphism of transcription factor 7 analogue 2(TCF7L2)gene and the risk of coronary heart disease(CHD)in type 2 diabetes mellitus(T2DM),and to find the susceptible genes and influencing factors of T2DM complicated with CHD.Methods A total of 100 T2DM patients were selected as T2DM group,100 CHD patients as CHD group,100 T2DM patients with CHD were selected as T2DM+CHD group,and 100 healthy persons were selected as control group.The polymorphism of TCF7L2 gene rs7903146 locus was detected in 4 groups,and the distribution characteristics of genotypes were analyzed.The differences of blood lipid and blood glucose among 4 groups were compared.The influencing factors of T2DM combined with CHD were analyzed by Logistic regression.Results The CC/CT genotype distribution at rs7903146 site of TCF7L2 gene was compared among the groups,and the difference was not statistically significant(P>0.05).There was no statistically significant difference in blood glucose and blood lipid levels between CC/CT genotypes in each group(P>0.05).Logistic regression analysis showed that aduanced age and high level of glycosylated hemoglobin were independent risk factors for T2DM and CHD,low levels of high-density lipoprotein cholesterol was a protective factor for preventing T2DM and CHD,and rs7903146 locus genotype was not an influence factor for T2DM and CHD.Conclusion The genetic polymorphism at rs7903146 of TCF7L2 gene has nothing to do with the risk of T2DM combined with CHD,blood sugar and blood lipid levels.
