摘 要:目的探讨微血管"珊瑚"成像技术(microV)在甲状腺结节微波消融术后随访中的应用价值。方法运用microV技术联合常规超声随访观察128个甲状腺良性结节消融后1 d、1个月、半年及1年消融灶内部及周边血流情况,比较microV与CDFI对消融灶血流的检出率;计算体积缩小率(VRR)并评估疗效,分析消融灶内部及周边血供在术后的变化及其对疗效的影响。结果 microV与CDFI对消融灶周边血流评级组间差异有统计学意义(P<0.05),microV的血流检出率更高;消融灶内部及周边血流均与随访时间存在关联性;单因素分析显示随访半年后内部0、1级组疗效较好(P<0.05),而周边血流3级组疗效较差(P<0.05)。结论甲状腺消融灶内部及周边血流均与疗效有一定的相关性,术后随访联合microV技术可更加准确评估血流,利于预估远期疗效,更利于医患间沟通。Objective To explore the application value of microvascularization(microV)technique in follow-up after microwave ablation of thyroid nodules. Methods MicroV technique combined with conventional ultrasound was used to observe the internal and peripheral blood flow of 128 benign thyroid nodules after ablation for 1 day, 1 month, 6 months and 1 year.Then, the sensitivity of microV and CDFI to the blood flow was compared, and the efficacy was evaluated according to the volume reduction rate. Finally, the postoperative changes of blood supply in and around the ablation lesions and its influence on the efficacy were analyzed. Results The difference between microV and CDFI in the rating of peripheral blood flow of ablation lesions was statistically significant(P<0.05), and the detection rate of microV was higher.The blood flow inside the ablation lesions and the blood flow around the ablation lesions were correlated with the follow-up time.Univariate analysis showed that the curative effect of grade 0 and grade 1 internal blood flow group was better(P<0.05), while that of grade 3 peripheral blood flow group was worse(P<0.05). Conclusions There is some correlation between the internal and peripheral blood flow in thyroid ablation lesions and the efficacy. Postoperative follow-up combined with microV technology can more accurately evaluate the blood flow, which is conductive to the estimation of long-term efficacy and the communication between doctors and patients.
