摘 要:新型冠状病毒肺炎流行给膀胱灌注的开展带来了新的挑战。在相关指导手册和指南的引导下,我们通过全预约就诊合理控制患者数目,通过体温监测和流行病学筛查初步筛选可疑新冠肺炎患者,采取尿常规检测—医生接诊—自助缴费—规范的药物准备过程—实施灌注—灌注后监测—膀胱镜复查等一系列流程,通过电子化、信息化方式减少患者聚集并简化就诊过程,并实施合适的防护措施保护医护人员和患者。自2020年2月17日至4月30日共为133例患者开展637例次灌注,平均每周57.9例次,平均每个出诊单元(8∶00~11∶30 am)29.0例次。无任何患者或医护工作人员出现疑似新型冠状病毒肺炎感染。所有患者无并发严重感染,无严重不良反应。故通过北京大学泌尿外科研究所(IUPU)管理经验,可以在新型冠状病毒肺炎流行期间实现安全有效的膀胱灌注管理。The COVID-19 pandemic brings new challenges to the implementation of routine bladder instillation.Under the guidance of relevant manuals and guidelines,the number of patients per unit was limited with appointment registration,the patients'temperature was monitored,and epidemiological information was collected to primarily screen suspected COVID-19 cases.A series of procedures including routine urine test,doctor's reception,self-service payment,standardized drug preparation process,implementation of bladder instillation,post-instillation monitoring,and cystoscope examination were adopted.Electronic and self-service methods were used to prevent patients gathering and to simplify the treatment process.The safety of healthcare workers and patients was ensured by providing adequate and appropriate personal protective equipment(PPE).From Feb.17 to April 30,2020,637 bladder instillations were carried out in133 patients,with an average of 57.9 times per week and an average of 29.0 times per visit unit(8∶00-11∶30 am).No severe infection,adverse event,or suspected COVID-19 infection we observed.The IUPU management experience indicated that safe and effective bladder instillation can be carried out during the COVID-19 pandemic.
