摘  要:目的回顾性分析普通型新型冠状病毒肺炎(COVID-19)患者临床症状与胸部CT所示炎症吸收速率的特点,以期对临床诊疗提供参考依据。方法对2020年2月18日~2020年3月26日因疑似患有普通型COVID-19而被收入武汉泰康同济医院轻症病房的97例患者,根据首诊CT表现分为实性型、混合型、磨玻璃型病灶三组,记录患者的一般情况、首发症状、基础疾病、影像学表现。肺部病变:测量普通型COVID-19患者入院后间隔7 d的两次胸部CT检查的病灶面积分别为S1、S2,计算病灶吸收百分比A=[(S2-S1)/S1]×100﹪,则病灶吸收速率为(V)=A/7。结果磨玻璃型炎症与实性型炎症V分别为0.0302,0.0695,两组间比较差异有统计学意义(P=0.007),而磨玻璃型炎症和混合型炎症组间、混合型炎症与实性型炎症组间比较炎症吸收速率差异无统计学意义(P=1.000,P=0.094)。结论①普通型COVID-19患者不同的影像分型中实性型炎症吸收速率明显快于磨玻璃型和混合型炎症;②根据不同分型的普通型COVID-19吸收速率计算出的总疗程,可预测患者出院后最终的复查时间点。Objective To retrospectively analyze the characteristics of the clinical symptoms and the rate of inflammation absorption shown by chest CT in patients with common new coronary pneumonia,in order to provide a reference for clinical diagnosis and treatment.Methods 97 patients who were admitted to the mild ward of the Wuhan Taikang Tongji Hospital because of suspected common new coronary pneumonia were enrolled.They were divided into three groups:solid,mixed,and milled based on the CT appearance of the first diagnosis.The general conditions,first symptoms,underlying diseases,and imaging findings of these patients were recorded.Thearea of lesions(S1 and S2)on two chest CT scans were measured,and the percentage of lesion absorption were calculated as A=[(S2-S1)/S1]×100﹪,then the rate of lesion absorption was calculated as V=A/7.Results The absorption rate(V)of milled inflammation and solid inflammation were 0.0302 and 0.0695,respectively.There was a significant difference between the two groups(P=0.007).There was no significant difference in the absorption rate of inflammation between the milled inflammation and mixed inflammation groups,and between the mixed inflammation and solid inflammation groups(P=1.000,P=0.094).Conclusions①The absorption rate of solid inflammation in different image classifications of patients with common new coronary pneumonia is significantly faster than that of milled and mixed inflammation.②The total treatment period calculated according to the absorption rate of different types of common new coronary pneumonia can predict the final review time point after the patient is discharged from the hospital.
